Outpatient Dietitian - Days

Position Highlights: Sign on bonus: None Position: Outpatient Dietitian - Days Location: Swedish Hospital, Chicago Part Time - 20 hours/week Hours: Monday-Friday, 8:00am - 4:30pm Required Travel: No What you will need: Bachelors of Science degree Illinois state license as a Licensed Dietitian Nutritionist Current registration as a Registered Dietitian by the Commission on Dietetic Registration Current certification by the Certification Board for Diabetes Care and Education as a Certified Diabetes Care and Education Specialist (CDCES) One year experience providing diabetes education What you will do: The Clinical RD, Outpatient focuses on diabetes education and care in both the inpatient and outpatient environment. Inpatient responsibilities include providing bedside education for admitted patients with uncontrolled or newly diagnosed diabetes, offering tools and resources for blood glucose management, and referring patients for outpatient follow up. Outpatient responsibilities include conducting individual diabetes education sessions with patients. The goal of this position is to bridge the gap between discharge to home and prevent avoidable diabetes-related readmissions. The RD is be expected to track the success and outcomes of this program. This is a temporary grant-funded position ? should financial funding for this role be discontinued at any time or cease beyond the duration of the grant period, the position may no longer remain active. Responds to inpatient consults for diabetes education, provides bedside education that includes offering tools and resources for blood glucose management and discharge education for newly diagnosed patients with diabetes. Maintains clinical competency which is tested each year by Nursing Education Performs 12-lead ECG on patients as directed by the RN Proactively organizes, and stocks nursing unit with patient care supplies. Maintains patient privacy and confidentiality Empty, measure and record output in all Foley bags/colostomy bags as directed by the RN and at the end of the assigned shift. What you will do: Performs duties of SANE with knowledge of forensic collection, STIs, medication and vicarious trauma towards the survivor. Attends SANE meetings for peer/chart review and information dissemination to ensure compliance and standards according to the Professional Forensic Scopes and Standard Practices, SASETA, and the Illinois Nurse Practice Act. Works collaboratively with the Swedish Hospital Emergency Department, Violence Prevention Program, Illinois State and Chicago Police Departments, Resilience, and the Illinois SANE Coordinator. Attends courtroom proceedings related to Swedish Hospital sexual assault cases. What you will need: Graduate from an accredited school of nursing, BSN preferred.
