The full-time schedule consists of working Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours based on client needs. Key Responsibilities\n \n Perform comprehensive evaluations of the psychosocial needs of students, particularly those facing emotional, behavioral, and social challenges. \n Collaborate closely with families, educators, and other professionals to develop tailor-made care plans that reflect individual goals and preferences. \n Provide expert counseling and emotional support to students, helping them navigate the complexities of school life and personal issues. \n Act as an advocate for students’ and families’ rights, connecting them to community resources and services that enhance their well-being. \n Engage in interdisciplinary team meetings with teachers, counselors, and healthcare providers to create a cohesive approach to student care. \n Conduct regular follow-ups to monitor students’ progress, home environments, and any emerging challenges that may arise. \n Facilitate effective communication among family members to address conflicts or support challenges in care. \n Educate clients and their families about resources, support groups, and programs available to enrich their emotional and social life. \n Maintain detailed documentation of client interactions, assessments, and care plans within our electronic health record (EHR) system. \n Continuously update knowledge of social work best practices, policies, and regulations, ensuring adherence to ethical standards in service delivery. \n Patient Population / Client Focus\n This position primarily focuses on providing support to students, including those from diverse backgrounds facing emotional, mental, and social challenges. By understanding the unique difficulties experienced by seniors and their families, the LCSW will play a pivotal role in delivering targeted interventions that empower clients to regain control over their emotional health. Qualifications\n \n A Master’s degree in Social Work from an accredited institution. \n Current and valid state licensure as a Licensed Clinical Social Worker (LCSW). \n A minimum of two years of applicable experience, ideally within a community service or geriatric-focused setting. \n Comprehensive understanding of social work principles, aging-related issues, and community resource availability. \n Demonstrated skills in assessment, counseling, and problem-solving. \n Empathy and interpersonal skills necessary to build rapport with students and their families. \n Strong communication skills to facilitate collaboration among diverse teams. \n Ability to effectively utilize electronic health record (EHR) systems and other relevant technology. \n Preferred Experience\n While a traditional clinical background is essential, experience working in educational settings or with senior populations is preferred.
