Job Summary Position Title: Medical Director – Intention MA Location: Greater Boston, Massachusetts Organization: Intention Healthcare Job Summary : Intention Healthcare is seeking an experienced and mission driven Medical Director to oversee and guide the Intention Provider team in delivering home-based primary care across Massachusetts. This leadership role ensures the delivery of high-quality clinical services through direct oversight of provider performance, documentation standards, quality initiatives, regulatory compliance, and clinical best practices. The Medical Director also serves as the primary clinical resource for the providers, offering mentorship, guidance and support for complex cases while fostering a culture of excellence, collaboration, and continuous improvement. The Medical Director will provide direct clinical care under a reduced patient panel. Responsibilities Responsibilities include: Clinical Leadership and Oversight · Provide clinical supervision, mentoring and ongoing support to Intention providers across Massachusetts · Serve as the primary point of contact for clinical questions, diagnostic guidance, and case consultations. · Ensure adherence to evidence based clinical standards, treatment protocols and organizational policies · Conduct periodic ride-alongs, case audits and chart reviews to monitor quality and consistency of clinical care Quality and Performance Management · Oversee provider performance metrics, including productivity, patient outcomes, documentation timeliness and quality, and safety standards · Lead quality improvement initiatives aimed at clinical effectiveness, patient satisfaction and decreasing utilization rates · Develop and implement workflows, best practices and quality benchmarks to optimize operational efficiency · Provide feedback, coaching and performance evaluations for providers including individualized improvement plans when needed. · Ensure closed loop clinical communication occurs with external clients, partners and care team members Program Development and Collaboration · Partner with multidisciplinary teams including physicians, PAs, NPs, specialists, behavioral health providers, and care coordinators to support integrated care delivery · Participate in strategic planning for clinical service expansion, program design and innovation in home based care · Support onboarding, training and continuing education for Intention Providers Clinical Practice · Conduct comprehensive health assessments during home visits, including physical exams, medication reconciliation, and review of medical histories. · Diagnose medical conditions, develop individualized care plans, and prescribe medications as appropriate. · Educate patients and families on disease management, lifestyle changes, medication adherence, and preventive care. · Maintain detailed and accurate electronic health records (EHR) of patient assessments, treatments, and follow-ups.
