Key Responsibilities\n \n Conduct comprehensive mental health assessments, including psychosocial evaluations and risk assessments tailored to the needs of patients in palliative and hospice care. \n Collaborate with patients and their families to create individualized treatment plans that address emotional, psychological, and spiritual needs during the hospice journey. \n Utilize evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT) in one-on-one, family, and group settings. \n Offer crisis intervention services in response to acute distress of patients and their family members, coordinating care with other healthcare professionals to ensure continuity and quality. \n Maintain precise and timely clinical documentation, including intake assessments, progress notes, and treatment plans, adhering to our regulations and organizational policies. \n Empower patients and families through psychoeducation, equipping them with strategies to navigate the emotional complexities associated with serious illness and loss. \n Regularly evaluate treatment outcomes and make necessary adjustments to enhance effectiveness, staying informed about the latest research and therapeutic trends within mental health. \n Adhere to ethical standards and confidentiality regulations, upholding the dignity and respect of all client interactions. \n Patient Population / Client Focus\n As a Licensed Clinical Social Worker at Family and Kids Behavioral Counseling Center, you will primarily focus on a diverse clientele facing end-of-life challenges.
