In this role, you will work closely with multidisciplinary teams, triage referred patients, facilitate timely and appropriate provider scheduling, and ensure continuity of care across outpatient and inpatient settings. The coordinator also serves as a liaison between referring providers, the interventional team, and patients, while supporting program growth through outreach and data management.
