The Crisis Stabilization Unit RN JOB PURPOSE/SUMMARY Summary of role of team : * Serves as a clinical member of the multi-disciplinary team, providing treatment, rehabilitation, and support for client population. * The CSU RN must be adept at multi-tasking. * Provides direct medical care to clients in a CSU setting including clients in varying levels of crisis, frequently working with SPMI and clients with co-occurring issues. * RN collaborates with Mobile Crisis and Walk in Center assisting with clients being referred to higher level of care, completing S-BARs. Summary of position : * This position requires the RN to provide direct medical care to clients, during 12-hour shifts, on a residential Crisis Stabilization Unit. * This position is required to provide nursing covering on a 12-hour shift despite any weather conditions or holidays that arise. * Required to be certified in and adequately implement verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, and going down on knees, running, and walking. TYPICAL WORKING CONDITIONS/ENVIRONMENT * Position is based within Crisis Services that provides services 24/7. * The role of the CSU RN is to ensure that coverage is provided 24/7 despite any weather conditions that may arise. * This position requires working different shifts as needed. * Must remain awake and alert during shift to monitor client safety. COMPENSATION: * Starting salary for this position is approximately $31.51/hr based on relevant experience and education. Daily Staff Duties: General * Attends treatment team as available. * Responds to all flags, emails, and voicemails within 1-2 business days. * Returns calls within 15 minutes. * Assists with administering medications and providing staff coverage as needed. * Clearly communicates to team members throughout shift regarding client behaviors, needs, and any other relevant information. * Works in a multi-disciplinary team to meet the needs of the clients served. * Demonstrates a consideration and concern for fellow workers and their jobs and promotes harmonious relationships and attitudes. * Willingness to accept additional assignments and / or changes in assignment and/or work duties. * Promotes an environment in which the customs and spiritual beliefs of the individual are respected. * Communicates directly and approaches conflict with a problem-solving approach. Crisis Unit * Performs the initial admission nursing assessment and completes required admission paperwork including nursing assessment, nursing note and nursing admission orders. * Update medication tabs. * Update allergy lists. * Complete paper MAR. * Obtain urine drug screen, BAL and pregnancy tests upon admission. * Follows all established protocols and guidelines for admission prior to admitting the client to the unit. * Makes the determination of the appropriate admission protocols needed for each client (ex.-agitation protocol, diabetic protocol, HTN protocol). * Obtains admission orders from the on-call prescriber in a timely manner. * Completes charting for each client during every shift prior to the end of shift. * Completes transition paperwork and discusses medication and aftercare instructions with client and/or family. Provides copy of paperwork for clients. * Counts controlled substances and documents at every shift change. Notify supervisor immediately of any discrepancies. * Obtains and charts vital signs on every shift. * Monitor for significant changes in client's condition/behavior every shift and make referrals to obtain new orders as needed. * Administer scheduled, routine, and PRN medication in a timely manner. * Obtains blood sugars as ordered. * Triages potential referrals with Mobile Crisis and Walk in Center. * Coordinates arrival times for potential clients with Mobile Crisis, Walk in Center and Transporters * Complete C-SSRS on each client * Attend to guest and/or client needs during medical emergencies (ie: seizures, falls, etc) * Coordinate with Emergency Departments and EMS * Obtain Prior Authorizations * Answer phone calls from recently transitioned guests regarding medication issues * Weekly medication counts of routine medications * Update Admission/Discharge log each time a client is admitted or transitioned from unit. * Complete follow-up calls on weekends in accordance to program requirements. Residential/Inpatient * Perform the initial medical assessment of individual within 30 minutes of arrival * Makes a determination of the appropriate protocols upon admission. * Completes all chart documentation within required periods and adheres to licensure agency, state and CARF Standards. * Monitors client's vital signs and record them in EMR. * Will administer medications accurately and safely following the 5 Patient Rights. * Work with multi-disciplinary team to meet client needs and maintain clinical standards of the program. * Ensure that the correct Standing Orders/Protocols are noted for clients being admitted * Complete all work assignment within shift worked * Attend all mandatory staff meetings. * Must be punctual and maintain a good attendance record. * Perform other duties as assigned by the Nursing Manager and Team Leader. Professional Development * Complete and maintain Relias Training Modules when required. * Maintain CPR and HWC certifications up to date. * Maintain and possess a current nursing licensure. Schedule: * Schedule requires 12 hours shifts, rotating weekends, and scheduled holidays. The hours are 7pm-7am, with 44 hours one week and 36 hours the next week. * Regular attendance is an essential job function. * Mandatory to remain awake and alert during shift. Travel : * N/A Equipment/Technical Competency : * Must be efficient in computer skills. * Use of Microsoft Office is required. * Must be able to use a fax machine and phone system. Equipment/Technology: * This position does require basic computer skills for timekeeping and use of electronic medical records entry. * Must be familiar with basic Microsoft Office Skills. QUALIFICATIONS - Crisis Stabilization Unit RN Experience / Knowledge: * Associate/Bachelor RN degree from an accredited College/University. * Experience in psychiatric nursing and acute care preferred. * Must have the ability to present professionally, work within a team format, and have the mental ability to exercise sound, clinical judgment under pressure. * Must have the ability to communicate effectively (both oral and written) and possess good time management and organizational skills. * Basic computers skills required; experience with electronic medical record (EMR) preferred. * Experience with interdisciplinary collaboration and care coordination preferred. The RN is to return calls within 15 minutes * Document administered medications in the EMAR Document clinical services provided in the medical record on same day as service provided in a clear and concise manner. 5. Completion of required HRMC training to maintain status as credentialed provider * Maintain an active RN license in the state of Tennessee in accordance with the state guidelines * Complete any Center Required Self-Directed Learning or Training Modules in the required time frames. * Complete First Aid, CPR, and HWC trainings within the required time frames. Schedule: * The Nurse Manager is expected to fulfill the agreed upon hours when hired. * Unless otherwise approved, the Nurse Manager is expected to be present in the clinic from 8am-5pm and is expected to complete a minimum of 16 hours of clinical work per week which may include nights and/or weekends. * Must be flexible with scheduling as the Nurse Manager will rotate being on call and is expected to cover open shifts in their assigned cohort on a PRN basis. * The Nurse Manager may also be asked to cover open shifts in the other cohort on a PRN basis. * All other scheduling arrangements are approved on a case-by-case basis.
