Key Responsibilities\n \n Conduct in-depth psychosocial assessments to identify and understand the nuanced mental health needs of patients and their families, ensuring informed and effective treatment planning. \n Facilitate individual therapy sessions using various evidence-based modalities such as cognitive behavioral therapy (CBT), solution-focused therapy, and trauma-informed approaches tailored to hospice care challenges. \n Develop personalized treatment plans that encapsulate therapeutic goals and actionable steps, modifying strategies as necessary based on ongoing assessments and patient feedback. \n Engage in multidisciplinary collaboration with psychiatrists, psychologists, and hospice staff to coordinate comprehensive care plans that address both mental and physical health needs. \n Provide crisis intervention and risk assessment when necessary, ensuring that the safety and well-being of patients remain paramount throughout their treatment journey. \n Document patient interactions and treatment outcomes accurately and in alignment with ethical guidelines, ensuring confidentiality and compliance with all legal standards. \n Lead group therapy sessions, psychoeducational workshops, and support groups aimed at promoting community among patients and families. \n Patient Population / Client Focus\n The LCSW will work closely with individuals and families navigating the complexities of life-limiting illnesses, providing psychological support during their hospice experience. Qualifications\n \n A master’s degree in Social Work from an accredited institution. \n An active state license as a Licensed Clinical Social Worker (LCSW). \n A minimum of two years of relevant experience in a mental health or outpatient environment, particularly in dealing with terminal illness or hospice care. \n Strong proficiency in evidence-based therapeutic modalities and a commitment to ongoing professional development. \n Exceptional communication, interpersonal, and assessment skills to build rapport with clients and collaborate effectively with a diverse team. \n Preferred Experience\n \n Familiarity with hospice care practices, including the psychosocial dynamics that impact patients and families facing terminal illness. \n Experience facilitating therapeutic groups or educational workshops focused on mental health within a hospice context. \n Knowledge and experience with electronic health records (EHR) systems. \n Compensation and Benefits\n The annual salary for this position ranges from $80,000 to $100,000, commensurate with experience and qualifications.
