Manages work assignment in effective and efficient manner: a) Reports any cases to the Hospice/Palliative Clinical Supervisor where goals can not be met or at-risk situations exist and responds appropriately. b) Remains flexible in daily planning, allowing time for emergencies or unexpected visits. c) Maintains productivity as established by the Agency. d) Utilizes payor sources correctly, reflecting changing levels of care. e) Delegates patient care responsibilities to Home Care Nurse (LPN) as appropriate. f) Participates in the development of the hospice interdisciplinary team care plan. 4. Establishes the plan of care to be delegated to the Homemaker/Home Care Aide/Companion, orients and supervises the home care aide in accordance with state and federal regulations and Agency policy: a) Develops an initial written plan of care for the HMK/HCA/Companion and updates this plan as patient's condition warrants but no less often than every 60 days. b) Obtains input from HMK/HCA/Companion regarding patient progress. c) Prepares written performance evaluations of home care aides under his/her supervision. d) Provides supervision of home care aides at a minimum of every two weeks or as required by payor source. 5. Develops and maintains sufficient professional skills and credentials to meet the needs of patient care: a) Attends a minimum of 12 hours of in-service per year. At least six of those hours shall deal with aspects of hospice care. b) Participates actively in self-appraisal and the formation of a plan of self-development. c) Knowledgeable of agency policies and procedures. Adheres to Agency Corporate Compliance Plan and HIPAA Privacy Plan as per written agreement. d) Seeks appropriate clinical input to ensure appropriate patient care. e) Projects professional image through attitude and compliance with dress code. f) Capable of precepting new nurses and students. g) Functions as a team member with consideration to all staff. h) Provides Agency with current copy of professional license. i) Maintains CPR certification. Ensures quality care is being provided through monitoring and tracking and using the information to improve the knowledge and skill of trauma care providers and promote performance improvement and patient safety within an established institution structure. · Participants in the preparation for all site visits and/or accreditations to ensure maintenance of requirements established by the State and the American College of Surgeons. · Assists in ensuring data abstraction and entry into the trauma registry database is accurate.
