General Summary of Position Responsible for the coordination of patient care across the continuum of Radiation Oncologic care for all patients. Educates/provides information, supports patients, and facilitates treatment plans. Functions as a patient advocate, guides patients and families throughout cancer care and survivorship experience. These functions are performed in accordance with all applicable laws and regulations and MedStar Georgetown University Hospital's philosophy, policies, procedures and standards. Primary Duties and Responsibilities A. Performs the intake, assessment/triage, coordination and navigation of all patients referred to for Radiation Therapy. B. Serves as a liaison for referring physicians seeking information, or access to the system. C. Screens patients for appropriate information and manages the acquisition of pertinent medical records for initial appointments. D. Responsible for the planning and implementation of the patient evaluation process. E. Facilitates the evaluation of patients by interfacing with referring physicians and MGUH services, including: Radiology, Medical Oncology, Radiation Medicine and other MGUH services, as needed. F. Coordinates the complex management of patients at various stages of their diagnosis, treatment and follow-up, collaborating with all disciplines involved in the patient's care, to assure timely progress toward the best clinical outcome in the most effective manner. G. Identifies opportunities for patient participation in clinical trials. Communicates information to physicians, research coordinators and referring physicians to promote clinical trial enrollment.
