Home Health Infusion Care Coordinator (Hiring Immediately)

What You Will Do: Maintains daily communication with case managers/discharge planners and units to help ensure that patients referred for services are followed up with in a timely and efficient manner. Responsible for the successful clinical coordination of care from the referral source into the home setting on behalf of Mount Carmel Infusion Services.
