To receive the full base salary the physician must produce 1 million in gross revenue for/to the company. - 3rd year 550,000. To receive the full base salary the physician must produce 1 million in gross revenue for/to the company, this will also allow physician consideration for a 3% ownership in the OBL.Other bonuses: - Sign on bonus of 50,000.00 - Relocation bonus of 12,000.00 - CME, License fees, Full health coverageVacation: - 3 weeks paid off (15 weekdays) - 5 days for CME Schedule/hours/call requirements: - Full Time - Permanent - Monday through Friday - 6 AM until 5 PM - Physicians will be assigned the responsibility for making up call schedules. Weekday and weekend coverage will be shared fairly between all members of the coverage group. - Some members may be assigned less than full coverage responsibilities based on their full-time equivalency within their practice these decisions will be at the discretion of Medical Director. - Schedules will be made up for three month periods at least two weeks prior to the beginning of a new quarter. The administrative secretary assisting the Medical Director will be responsible for typing and distributing Call Schedules. Call is defined as after hours and weekend answering service calls, ie: calls from hospital ER asking for patient history or advice on a current clinic patients or new referrals, currently no STEMI call. Typical number of calls in a week are 15-30.
