The Opportunity: We are currently seeking a certified, or soon to be certified, Family Medicine Physician who is interested in joining an employer-based One Medical clinic that is committed to changing the care delivery experience for both patients and providers. One Medical is com mitted to fair and equitable compensation practices. The base salary range for this role is $308,700 to $328,000 based on a full-time schedule. Final determination of starting pay may vary based on factors such as practice experience and patient care schedule. Reporting to the Tele-Assist Program Operations Manager, Field Care Coordinators go into patients’ homes to facilitate telehealth visits with a provider in order to reach more of our patients where they live. This will include building a trusted and supportive relationship with the patient and their family, performing clinical assessments, assessing home safety, documenting medications and reviewing provider care plans with the patient. You will also assist in teaching patients and family members to use technology to coordinate care with their care teams, as needed.
