RN will be assigned to a very small territory consisting of 3 zones or zip codes clustered nearby their primary residence (RN will NOT cover the whole borough) Territory: NORTH Brooklyn NY - Field / Community based Comprised of the neighborhoods Bedford-Stuyvesant, Bushwick, Greenpoint, and Williamsburg, this area of Brooklyn is a hotbed of arts and culture.) Work Schedule Monday through Friday 830 AM to 5 PM - 4 Hospice visits per day. You can do all your charting and care planning calls from your HOME office in the afternoons as patient visits are usually done in the morning. You will only see FOUR Hospice patients per day and will manage caseload of terminally ill, end-of-life patients, delivering skilled nursing care in home and facility settings, supporting patients and families throughout the end-of-life process Create and follow plan of care to provide patients with Maximum level of comfort and quality of life. RN will be responsible for the overall case management of assigned patient caseload through the coordination of hospice services as determined in the Hospice Plan of Care. Provide patient physical and psychosocial health systems assessments for the purpose of developing the Hospice Plan of Care and “Goals of Care” based on the patient’s and family’s needs and wishes. Follows infection control standards as per company policy. Educate patients and families on disease progression, symptom management, and medication administration Collaborate with Physicians to regularly titrate medications and update individualized care plans while simultaneously working with IDT team in developing comprehensive, patient-centered plans of care Assess and make recommendations for medical equipment and/or supplies and evaluate equipment for safe use. Coordinate with office staff or vendor for delivery of equipment or supplies, anticipate need for supplies prior to weekends or holidays, and maintain proper inventory levels in the home.
