RN - Discharge Unit
Overview The Discharge Unit RN provides care for the adult patient population ready for discharge. This may include but is not limited to acute medical, surgical, neuro, and cardiac diagnosis who are medically stable and cleared for discharge by the primary care team. Qualifications Certifications: Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR); Basic Life Support (BLS) within 30 days - American Heart Association (AHA), Education: College Diploma: Nursing, Work Experience: Responsibilities Promotes the use of evidence-based practice through the nursing process, protocols and procedures. Assesses patient's status through interview, observation and physical assessment of actual problems and outcomes. Assesses and chooses teaching strategies appropriate to the patient's/family's age specific needs. Adapts teaching strategies based on patient's response, readiness to learn and level of comprehension. Takes cultural, age-specific, gender, and developmental factors into consideration when teaching patients about procedures, illness, hospital experience, etc. Identifies and utilizes team resources to solve identified problems or needs. Consults appropriate resources within the hospital to provide specialized education. Completes required documentation as appropriate, including patient chart and event reports. Analyzes clinical situations in a systematic way and acts to obtain resources or correct problems to meet or exceed expected outcomes or patient needs. Advocates with physicians on patient's behalf when necessary. Promptly and efficiently follows up on patient/family requests/needs - anticipates patient/family needs. Recognizes need for assistance and utilizes resources, including physician and nurse to nurse consultation in a timely manner. Demonstrates technical knowledge of equipment and troubleshoots when problems arise. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
