Position Summary The Float Pool Licensed Practical Nurse (LPN) is a member of Levindale's Staff Resource Pool and provides nursing care across multiple clinical areas based on operational and patient care needs. Working under the supervision of a Registered Nurse and/or Director of Nursing, the Float Pool LPN delivers high-quality, resident-centered care while demonstrating flexibility, adaptability, and strong clinical skills in a variety of patient care settings. Float Pool LPNs support multiple specialty areas throughout the hospital, helping ensure continuity of care and operational excellence across the organization. Essential Responsibilities Deliver safe, compassionate, and evidence-based nursing care across assigned units. Conduct ongoing patient assessments and collect relevant clinical data. Obtain and document patient histories, perform focused assessments, and complete admission documentation according to established policies. Administer medications, treatments, and procedures in accordance with physician orders and nursing standards. Monitor patient conditions and promptly communicate significant changes to the Registered Nurse and interdisciplinary team. Assist in developing, implementing, and evaluating individualized care plans. Provide patient and family education regarding treatments, medications, and care plans. Identify and address patient and family cultural, emotional, spiritual, and psychosocial needs. Collaborate with physicians, nurses, therapists, and other interdisciplinary team members to promote optimal patient outcomes. Maintain accurate and timely documentation within the electronic medical record. Participate in quality improvement initiatives, staff meetings, and professional development activities. Demonstrate flexibility and adaptability while floating to various units throughout the facility. Support daily unit operations and assume shift leadership responsibilities as assigned. Work every other weekend as required. Qualifications Education Graduate of an accredited Practical Nursing Program required. Associate Degree in Nursing preferred. Experience Minimum of two (2) years of nursing experience required. Experience in post-acute care, rehabilitation, skilled nursing, long-term care, or behavioral health preferred. Licensure & Certifications Current Maryland Licensed Practical Nurse (LPN) License required. Basic Life Support (BLS) certification through the American Heart Association required. As a Home Health Occupational Therapist , you will: Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives. Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist the physician in evaluating the patient's level of function. Confer with the patient's physician and other health care team members and participate in development/revision of the Plan of Care Treatment. Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures. Review/expand the retraining programs in a manner consistent with the behavioral goals of each patient and within the guidelines of the rehabilitation program as a whole. Consult with other vocational team members, as appropriate, to develop and implement vocationally oriented plans consistent with the needs and capabilities of patients. Report patient status and progress to the physician, rehabilitation staff, Clinical Manager, patient and family members. Participate in care coordination and discharge planning activities and act as a resource to other health care personnel in meeting patient's needs. Design community reintegration activities to assist the client in the physical reconditioning effort, and/or the psychological adjustment and coordinate the plan with members of the interdisciplinary team. Recommend and/or design special adaptive equipment for clients to improve residence or working environments or improve their participation in the rehabilitation program and/or community. Design/train staff and family members to carry out the retraining program including dressing, feeding, grooming and hygiene skills, participate in team and family educational meetings. Supervise, instruct and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services. Accurately and thoroughly document patients' care observations, interventions and evaluation on the day services are rendered. Ensure that interim (verbal) orders received from the physician are accurately documented and implemented. Submit evaluation, treatment plans and discharge summary to the supervisor and care management staff.
