Scope of Position The Medication Management / Transitions of Care (TOC) Pharmacy Technician provides a valuable patient-centered service by focusing on facilitating TOC, including obtaining and documenting a complete and accurate admission medication history and allergy information in the electronic medical record (EMR) as well as assisting with the discharge medication process. The TOC Pharmacy Technician completes the admission medication history during an inpatient admission, ambulatory encounter, pre-admission testing, surgery encounter, or within a procedural area under the supervision of a pharmacist, physician, or advance practice provider. Information can be gathered from a face-to-face or telehealth interview with the patient chart and communication with patients, caregivers, physicians, retail pharmacies, and long-term care facilities.
