The VA San Diego Healthcare System is looking to add a Primary Care Physician to our team at our Oceanside Community-Based Outpatient Clinic (CBOC). The CBOC operates under a Patient Aligned Care Team Model. Together with their team, comprised of a PCP, RN, LVN, and an administrator, the PCP cares for a panel of approximately 1,200-1,260 patients. The PCP provides primary care services in a team-based setting, including chronic disease management and preventive health. Common diagnoses encountered include diabetes, hypertension, chronic obstructive pulmonary disease, degenerative arthritis, depression, congestive heart failure, and chronic pain. The physician evaluates, diagnoses, treats, and provides consultation for non-surgical adult patients of all ages, without life-threatening complications and with a wide variety of diseases, injuries, and disorders. He or she assesses, stabilizes, and determines the disposition of patients with emergent conditions consistent with medical staff policy regarding emergency and consultative call services. The physician may collaborate with /oversee mid-level providers such as nurse practitioners and work collaboratively with other specialties (and subspecialties) in and outside the VA system to manage or co-manage patients' health. Select mental health and subspecialty services are available on-site. Other mental health, medical and surgical subspecialties services are available through consult with the San Diego VA Health Care System or to the community. If there is a complication in the course of a patient's care, the staff podiatrist will discuss the untoward event, including the patient's options for a second medical opinion or legal action, and will document these discussions in the chart (CPRS). Report administratively and clinically to the Chief of Podiatry. In addition, this individual may report to the Chief of Surgery for the program(s) to which he/she is assigned. Position Highlights: D ell Children's Medical Group Rheumatology has three attending physicians and one PA (looking to add a fourth attending). Our goal is to have four Pediatric Certified Rheumatologists in clinics covering Central Texas from Buda to Cedar Park. We offer embedded physiology and psychiatry in our clinics. Ultrasound in the outpatient clinic for joint injections and two locations with dedicated Rheumatology infusion suites.
