Care Coordinator

POSITION SUMMARY/RESPONSIBILITIES Care Coordinator will be instrumental in assisting the department and clinicians in the Ambulatory setting by gathering information, coordinating utilization efforts, and reviewing HCC quality indicators, and RAF scores to eligible Medicare Advantage beneficiary. Will monitor opportunities within the Medicare managed group to enhance financial outcomes. Will coordinate the transition of care and the interdisciplinary treatment for Medicare managed patients across the healthcare continuum. Facilitates the delivery of services, evaluates effectiveness, tracks outcomes and functions as the patient advocate to identify and communicate health care needs. Works collaboratively with clinical staff, clinic leadership, and outside agencies in an effort to improve patient outcomes, compliance, and decrease complications. EDUCATION/EXPERIENCE Graduation from an accredited school of nursing with current RN licensure in the State of Texas, BSN preferred. Three years recent, full time hospital experience preferred. Work experience in case management, utilization review, or hospital quality assurance experience is preferred. LICENSURE/CERTIFICATION Current license from the Board of Nurse Examiners of the State of Texas to practice as a registered nurse is required. National certification in related field is desirable. Case Manager Certification (CCM, CPHQ, or ANCC) or Certified Diabetes Nurse Educator certification is highly desirable. Must have a current AHA BLS Healthcare Provider or AHA BLS Instructor Provider card.

Specialty Nurse Practitioner Outpatient Interventional Cardiology

Position Summary The Advanced Practice Provider (APP) will provide comprehensive outpatient care to patients under the supervision and collaboration of interventional cardiologists. The APP will evaluate, diagnose, and manage patients with cardiovascular disease, focusing on continuity of care before and after interventional procedures. This role emphasizes outpatient clinic management, coordination of patient care, and integration with inpatient and procedural services to optimize outcomes and patient satisfaction. Key Responsibilities Clinical Care Conduct new patient consultations, follow-up visits, and post-procedure evaluations in the outpatient clinic. Perform focused histories, physical examinations, and order/interpret diagnostic test results (e.g., echocardiograms, stress testing, vascular studies, labs). Supervise cardiac stress tests (training provided if not experienced) Manage cardiovascular risk factors and chronic conditions (hypertension, hyperlipidemia, CAD, PAD, heart failure) under established protocols. Provide pre-procedure evaluation and education prior to interventional cardiology procedures. Deliver post-procedure follow-up, including medication adjustments, wound care, and complication monitoring. Collaborate with supervising physicians in the formulation and execution of treatment plans. Care Coordination Facilitate seamless transitions of care between hospital, cath lab, and outpatient clinic. Communicate with referring physicians and primary care providers regarding patient progress and treatment plans. Assist with triage of referrals to the interventional cardiology clinic, prioritizing urgent cases (e.g., unstable angina, critical limb ischemia). Coordinate acquisition and review of outside medical records and imaging. Education & Counseling Educate patients and families on disease processes, procedures, lifestyle modification, and medication adherence. Provide counseling on cardiovascular prevention and rehabilitation strategies.

Nurse Clinical Transplant Coordinator

POSITION SUMMARY/RESPONSIBILITIES Under general supervision, the Clinical Transplant Coordinator I will use clinical management, consultation, education and research to promote quality care for the specific transplant patient populations. Supports and maintains the University Health System's policies, protocols, values and guest relations. Directly accountable to the Director and the Supervisor, Clinical Transplant Nurse. May direct activities and services provided by personnel in assigned area, such as Licensed Vocational Nurses, non-licensed nursing and secretarial support staff. EDUCATION/EXPERIENCE Graduation from an accredited school of nursing with an RN and one year of nursing experience required. Will develop a plan to attain a Bachelor's Degree in Nursing. Knowledge of transplant, health care trends, community and regional resources. An ability to establish cooperative working relationships with diverse groups and individuals, medical staff and other health care disciplines and understanding of the consultative process. LICENSURE Current RN licensure in the State of Texas. Current American Heart Association, Basic Cardiac Life Support Healthcare Provider certification required. Attain CITI research certification within six months from date of hire. (CITI certification is not required for coordinators in lung transplant, post-transplant, live donor transplant or disease management nurses).

Staff Nurse II

POSITION SUMMARY/RESPONSIBILITIES Trains patients and care partners on self-care home hemodialysis (HHD) or peritoneal dialysis (PD) techniques, equipment use, infection control, and emergency procedures. Home Dialysis nurses will also conduct home visits to assess the environment and ensure safety and compliance. Home Dialysis nurses will also be required to take on call responsibilities for patient and care partner support during and after office hours, and on holidays. Performs the nursing process in a safe therapeutic manner in a designated clinical setting. Provides direct patient care (HHD/CAPD/CCPD) and monitoring of renal dialysis patients. Coordinates home dialysis activities within the frame work of the Federal Regulations, State and Local End Stage Renal Disease Network. Supports and adheres to University Health and department specific policies and standards. Receives mentoring from the Patient Care Coordinator and Director of Dialysis Services. EDUCATION/EXPERIENCE Registered nurse who has at least 2 years of experience as an RN, and six months experience in the specific home dialysis modality. BSN is highly preferred. Must meet career ladder Staff Nurse II criteria for specialty area worked. National Certification in Dialysis is preferred. Must have current American Heart Association, Basic Cardiac Life Support and Healthcare Provider card. LICENSURE/CERTIFICATION Must be a Registered Nurse currently licensed in the state of Texas.
