Direct Patient Care Responsibilities Maintains an active patient panel consistent with organizational access, panel size, and productivity expectations and provides the full scope of preventive and primary care services within professional licensure and scope of practice, including: Establishes and maintains therapeutic relationships with patients. Ensures informed consent and patient understanding of diagnoses and treatment plans. Provides chronic disease management and preventive care consistent with evidence-based guidelines. Integrates assessment of social determinants of health into care planning and collaborates with care teams to address barriers to care. Supports access initiatives, including same-day care and panel management strategies. Refers appropriately to internal programs (dentistry, behavioral health, diabetes education, OBAT, SRH, etc.) and external specialty providers. Works within a team-based care model, collaborating with nursing, care management, behavioral health, and support staff to optimize patient outcomes. Documents all encounters in a timely, accurate manner reflecting medical necessity and supporting appropriate coding and billing. Submits billing documentation within required timeframes (within 48 business hours of visit). Participates in 24-hour on-call rotation as scheduled (approximately four times per year), providing telephone back up to the nurse triage service. Transfers care appropriately when patient needs exceed professional scope, skill, or comfort. Quality Improvement & Compliance Participates in departmental and organizational meetings as required.
