JOB DESCRIPTION Job Summary Provides support for care management/care coordination activities and collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum. Strives to ensure member progress toward desired outcomes and contributes to overarching strategy to provide quality and cost-effective member care. Essential Job Duties • Completes assessments of members per regulated timelines and determines who may qualify for care coordination/care management based on triggers identified in assessments. • Develops and implements care plan in collaboration with member, caregiver, physician and/or other appropriate health care professionals and member support network to address member needs and goals. • Conducts telephonic, face-to-face or home visits as required. • Performs ongoing monitoring of care plan to evaluate effectiveness, document interventions and goal achievement, and suggest changes accordingly. • Maintains ongoing member caseload for regular outreach and management. • Promotes integration of services for members including behavioral health, long-term services and supports (LTSS), and home and community resources to enhance continuity of care. • Facilitates interdisciplinary care team (ICT) meetings and informal ICT collaboration. • Uses motivational interviewing and Molina clinical guideposts to educate, support and motivate change during member contacts. • Assesses for barriers to care, provides care coordination and assistance to member to address concerns. • Collaborates with licensed care managers/leadership as needed or required. • 25- 40% estimated local travel may be required (based upon state/contractual requirements). Clears jams from bodymaker and trimmer Conducts daily or monthly lubrication on ironers for normal operation of equipment. Identifies trim quality problems. Checks three cans of trimmer every two hours for dimension Identifies the proper size rings to ensure customer specifications are met Sets timing for cup infeed, can blowoff and discharge pocket for bodymaker Operates the ironers and trimmers during daily production runs. Adjust trim heights to Crown’s specifications for finish. After the initial training period, this position will oversee production on third shift. • Conduct various safety audits of equipment and procedures along with conducting safety observations. • Inspect equipment and product to ensure optimum safety, utilization and quality. • Communicate with all employees concerning safety, staffing needs, scheduling, problem solving, change actions, overtime, vacations, and training. • Review and support master scheduling process to meet production needs. • Plan process activities and follow established priorities regarding procedures, equipment capabilities, process team performance and staffing levels. • Review, analyze and act to correct non-conformance in areas of safety, policies, and procedures. • Lead, coach, and support line process teams in their efforts to improve quality, increase flexibility and reduce costs. • Conduct required team meetings and safety training. • Maintain time and attendance records, as well as production data in electronic systems. • Provide leadership for hourly employees and hold them accountable to achieve departmental and company goals. • Provide feedback to hourly employees on job performance. • Coordinate manufacturing activities to insure production and product quality meet specifications. • Interface with internal and external vendors to provide parts, support, and training as it relates to production and maintenance. Position Overview: Ensure Printers operate efficiently with required supplies available to produce aluminum cans at the highest quality standards to meet daily customer schedules. Duties and Responsibilities: Safety & Quality minded with willingness for excellence required. Ensures all safety equipment, guards, and devices are always in place, and follows safe work procedures and safety rules. Verifies proper setup of printer equipment, has the required tools for the job in good condition (see Group 7 Tool List). Operates the printer while checking for quality defects in the manufacturing process. Performs label design changeover setups. Performs timely quality control checks to ensure printer and related equipment is within Specifications. Audits printer applications to ensure SOP specifications are met. Must work effectively in a team environment and be self-driven by working independently. Analyze spoilage, record causes, and take corrective actions where applicable. Performs preventative maintenance assignments. Monitors equipment or processes and reports all problems to supervision. Follows all SOP operating procedures Keeps the area safe, clean and organized. Performs other related duties as required. Qualifications: In addition to the specific responsibilities listed above, the ideal candidate will possess the following: Minimum Requirements 2-4 years manufacturing/production experience. Competency Strong computer skills and working knowledge of various Microsoft programs (Excel, Word, Access, etc.). Physical Requirements While performing the duties of this job, the associate is regularly required to sit, stand, walk, use hands, reach with hands and arms, and may be required to balance, stoop, kneel, crouch, or crawl. Working Conditions Generally, works in a plant environment but will be required to perform some job duties inside a typical office setting.
