What You Will Do: This is a mobile position involving daily travel to patients' homes throughout Las Vegas/Henderson. You act as the primary clinical point of contact for your patients. Execute physician plans of care for patients recovering from illness or managing chronic disease. Perform skilled interventions including complex wound care, infusion therapy, and medication management. Accurately complete OASIS data collection at required time points. Coordinate directly with our Clinical Director and therapy teams to ensure positive patient outcomes. Job Description Job Description Perform ongoing nursing assessments appropriate to the nurse’s licensure and assigned responsibilities. Monitor vital signs, oxygen saturation, respiratory status, neurological status, hydration, nutrition, and skin integrity. Recognize early signs of infection, seizures, respiratory compromise, dehydration, and other changes in condition. Implement ordered interventions and notify the appropriate healthcare professionals of significant findings. Administer prescribed medications accurately and safely through authorized routes, including oral, G-tube, inhalation, topical, and other routes permitted by the nurse’s license and care plan. Follow medication administration safety standards, including patient identification, medication verification, dose, route, time, documentation, and response monitoring. Document intake and output, feeding tolerance, and relevant gastrointestinal assessments. Respond to G-tube complications according to the patient’s individualized care plan, provider orders, and agency protocols. Perform routine tracheostomy care per physician orders and the established care plan. Assess respiratory status, including respiratory rate, effort, oxygen saturation, breath sounds, and airway patency.
