Requirements MINIMUM QUALIFICATIONS: Graduate from an accredited school in medical assisting; Three (3) years’ experience working as a Medical Assistant in a physician/hospital setting may be substituted in lieu of graduation from an accredited school in medical assisting. Certified in BLS/CPR. Demonstrated excellence in customer service, as well as good communication and interpersonal skills. PREFERRED QUALIFICATIONS: One to two years’ experience as a Medical Assistant in a physician office setting. Basic computer keyboard knowledge. Flexibility to adjust to schedule changes. Care Coordinator - Community Prevention & Treatment Services

GENERAL RESPONSIBILITIES: The Care Coordinator is a member of the Community Prevention and Treatment Services Team. This position works directly with patients who are living with infectious diseases and experiencing barriers to care and treatment. Populations served by CPTS Care Coordinators consist primarily of individuals facing social health disparities. This includes but is not limited to individuals living with HIV and/or Hepatitis C, individuals living with substance use disorder, members of the LGBTQIA community, and individuals seeking gender-affirming care. These services are provided to patients regardless of race, ethnicity, age, income status, gender expression and/or orientation, and sexuality. The CPTS Care Coordinator is responsible for assessing the needs of the patient throughout all stages of infectious disease medication management. This includes developing an individualized service plan, coordinating service delivery to meet the patient’s self-identified goals and addressing patient-specific challenges. The CPTS Care Coordinator participates in multidisciplinary team meetings in partnership with clinical staff to discuss patient needs on a weekly basis. The role of the Care Coordinator is to advocate on behalf of the patient and act as liaison to internal/external organizations providing whole-person care services. SPECIFIC RESPONSIBILITIES: Actively manages a caseload of individuals living with infectious diseases. Develops comprehensive, individualized care plans and coordinates service delivery and activities required in implementing these care plans. Conducts face-to-face or over the phone intakes with individuals who are HIV or Hepatitis C positive in need of treatment/medication management. Performs Social Determinant of Health and Mental Health assessments with patients on a regular basis. Responsible for coordinating all aspects of medical care for members of caseload, including transportation to medical appointments, scheduling external specialty appointments, and any other patient-specific needs. Demonstrates knowledge of development across the lifespan, cultural and linguistic norms of minority populations and subpopulations, and awareness of socioeconomic health disparities as it pertains to medical and mental health treatment and prognosis. Interprets age-specific and cultural responses to treatment to improve health outcomes. Provides education, encourages retention in care, adherence to medical treatment, and strategies for promoting chronic disease self-management. Identifies the appropriate time for integrating family or the patient’s informal support network to help achieve clinical objectives. Manages referrals to appropriate agencies required to assist the client in achieving the goals and objectives identified in the Service Plan. Participates in case conferencing with the other members of the CPTS clinical team, as appropriate, and as required by acuity level. Participates in weekly HIV/HCV/PrEP Clinical Care Review Assist clients to access programs that will help pay for medical care and prescriptions. Conducts client-specific advocacy related to supportive services. Reviews clients’ utilization of services. Participates in outreach and engagement activities. Oversees transfer, inactivation, and discharge processes. Ensures timely documentation in the electronic health record and in designated State and Federal data management systems. Participates in Performance Improvement/Continuous Quality Improvement activities, as assigned. Demonstrates excellence in both internal and external customer service. Behavioral Health Clinician II - LCSW

GENERAL RESPONSIBILITIES: The Integrated Behavioral Health Clinician participates as part of a multidisciplinary team in an integrated behavioral healthcare setting providing a recovery oriented system of care to persons with substance use, mental health disorders and chronic co-morbid health issues. This position plays an integral role in integrating services to successfully ensure that individuals across the health-care domains receive all services necessary to experience optimal physical and mental health. The Integrated Behavioral Health Clinician conducts screenings and assessments, provides evidenced individual, and family and group therapy and individualized treatment planning that addresses integrated health care needs. The Integrated Behavioral Health Clinician is responsible to ensure that there is swift access to care, that performance outcomes are met and; patients experience satisfaction with services (Triple AIM). The Integrated Behavioral Health Clinician recognizes and respects the inherent rights of privacy, confidentiality, dignity and respect of all persons. Coordinates and consults with other staff and service providers to support the best the attainment of treatment goals and positive outcomes. SPECIFIC RESPONSIBILITIES: Conducts face-to-face comprehensive biopsychosocial assessments and mental status exams, as well as a diagnostic assessment for all clients initiating care, and reassesses patients on an ongoing basis. Develops a comprehensive, individualized treatment plan in collaboration with the multidisciplinary team and client. Oversees the periodic re-evaluation and revision of the treatment plan as necessary over the life of the client’s care. Exhibits knowledge and skill in all functions of the electronic medical record pertinent to successful job performance. Provides evidenced based individual, group, and family counseling as assigned. Provides crisis intervention services as indicated. Provides case management services as necessary and ensures coordination of care amongst service providers. Maintains and protects accurate and timely documentation in case records according to agency policy and regulatory requirements. Facilitates the collaboration between primary care and behavioral health care that includes participating in multidisciplinary collaboration toward the achievement of behavioral and physical health care outcomes. PrEP Specialist

This position works directly with patients to educate them on the benefits and administration of Pre-Exposure Prophylaxis (PrEP) in the interest of remaining HIV negative. Populations served by CPTS PrEP Specialist consist primarily of individuals facing social health disparities. This includes but is not limited to individuals living with substance use disorder, members of the LGBTQIA community, people of varying disability status, and individuals seeking gender-affirming care. These services are provided to patients regardless of race, ethnicity, age, income status, gender expression and/or orientation, and sexuality. The PrEP Specialist is responsible for assessing the needs of the patient throughout all stages of PrEP medication management. This includes educating individuals on the importance of sexual health and awareness, risk reduction, and adherence counseling. The PrEP Specialist ensures that patients are effectively linked to care, receive routine lab work confirming their HIV negative status while on PrEP, and are offered testing for sexually transmitted infections on a quarterly basis. This position conducts community outreach to promote the importance and overall use of Pre-Exposure Prophylaxis. The role of the PrEP Specialist is to provide whole-person centered care to patients who are prescribed this preventative medication while promoting the message that PrEP is for everyone. RESPONSIBILITIES: Supports enrolled PrEP participants in medication adherence and appointment attendance. Coordinates face to face or over the phone intakes with individuals who are interested in PrEP services. Promotes a holistic approach to primary care by assessing needs through quarterly Social Determinant of Health, Mental Health, substance use, and program re-assessment screenings. Responsible for coordinating care for PrEP patients, including appointment scheduling, transportation to medical appointments, scheduling external specialty appointments, insurance navigation, and any other patient-specific needs. Coordinates and collaborates with local pharmacies to reduce barriers for timely access to affordable PrEP medication, including copay assistance programs for underinsured or uninsured individuals. Regularly performs outreach and provides PrEP and sexual health group education to populations receiving services from local community based organizations as well as staff at these organizations. Provides rapid HIV and Hepatitis C testing to individuals during outreach sessions at local CBOs if requested, ensuring that individuals are aware of their status and linked to care and treatment if necessary. Leads discussions virtually/in person about PrEP education and its effectiveness at both community and state levels. Responsible for ethical documentation of all patient visits, care coordination services, and lab results in both Athena and AIRS, including PrEP education, adherence, side effects, and risk reduction. Demonstrates knowledge of development across the lifespan, cultural and linguistic norms of minority populations and subpopulations, and awareness of socioeconomic health disparities as it pertains to medical and mental health treatment and prognosis. Responsible for submitting monthly program reports highlighting outreach activities, referrals made to the PrEP program, active engagement services, and program successes and challenges. Deliver in-house PrEP educational program for providers and clinic staff on an ongoing basis with emphasis to barriers to PrEP uptake. Develop PrEP educational and promotional/outreach materials in collaboration with Development staff for community residents, WMY patients, and other community partner agencies. Participate in provider meetings to address barriers in identifying and enrolling high-risk individuals who are appropriate for PrEP. Attend off-site community events; including weekends, to promote PrEP services at WMY Provide comprehensive care coordination services for patients with emphasis on barriers to treatment adherence. Routinely assess the need for partner services, including partner notification assistance, and make referrals as needed. Participates in monthly CPTS team meetings to measure progress towards accomplishment of work plan goals and objectives. Participates in Performance Improvement/Continuous Quality Improvement activities, as assigned.
