Conducts psychosocial assessments by interviewing the patient and their significant others to obtain history and identify preliminary issues for treatment focus. Contacts appropriate previous and current tertiary caregivers to obtain relevant collateral clinical data that may be needed for the patient's coordination of care, treatment planning and discharge planning. Develops, implements and monitors progress of the patient's individualized treatment plan. Provides individual, group and family process-oriented counseling sessions. Conducts educational, after care support and other didactic groups for patients and family members using various professional treatment modalities. Participates in regular clinical team meetings/staffing and provides input on the psychosocial, disease management and case management needs of the patient. Demonstrates an understanding, and adheres to all procedures and legislation, which apply to patients cared for in a Mental Health and Substance Abuse treatment facility. Demonstrates a comprehensive knowledge of community resources (12 step support meetings, recovery homes and residential treatment facilities) and medical allied staff expertise to assure an appropriate discharge liaison is implemented. Reports to community/state agencies any patient issues that are required by hospital policy and state or federal law. Serves as the facilities representative in legal proceedings resulting from said reports. Maintains accurate and timely medical records including psycho-social assessments, family session and daily progress notes to include school personnel and other community collateral involvement. Provides descriptions of therapeutic interventions and patient response for discharge planning process.
