Join the Heartbeat of Compassionate Care: Hospice RN Opportunity Job Description: - Deliver skilled nursing and dedicated care to patients and families in their homes through a collaborative, team-focused approach. - Conduct comprehensive assessments, evaluations, and plans based on patients' physical and psychosocial conditions. - Ensure exceptional continuity of care through effective communication and coordination with other health disciplines, community support systems, and social agencies. - Provide top-notch symptom management and promote optimal health and quality of life through teaching, counseling, and appropriate referrals. - Maintain accurate and complete patient records in line with agency standards and regulatory requirements. - Contribute to team scheduling and participate in on-call rotations, assessing patient needs over the phone and making home visits as necessary. - Engage in quality improvement initiatives and attend in-service training sessions to enhance your professional growth. - Collaborate with Home Health Aides and CNAs, ensuring compliance with care plans and quality standards. - Perform other related duties as assigned by your supervisor. Qualifications: - Current RN license in Montana (BSN preferred). - Valid driver’s license and reliable transportation. - Minimum of 2 years of recent acute care, home care, or hospice experience. - Proficient in clinical skills: venipuncture, catheterization, wound care, and parenteral therapy. - HCHB experience strongly preferred.
