Position Highlights & Primary Responsibilities: Provide daily rounding on patients in skilled nursing facilities (SNF) and long term care (LTC) settings. Serve as the primary provider for one to two SNFs, managing patients across both skilled and long term care levels. The typical combined daily skilled census across one to two SNFs ranges from 8-13 patients.
