Job Responsibilities: · Receives referrals and assesses in a timely manner those patients and families/significant others who are referred to social work service. · Integrates social work plan into overall patient care through interdisciplinary collaboration. · Provides skilled Social Work services in the areas of comprehensive case management, discharge planning, continuing care services, advocacy, clinical social work services, including crisis intervention, and patient/family education utilizing individual and family modalities. · Provides skilled social work services in the areas of abuse and neglect, advanced directives, psychiatric placements/issues, end of life issues pertinent to the assigned patient population. · Understands and utilizes hospital and community-based resources and entitlements. · Refers patients, families/significant others, and hospital staff to appropriate services to ensure continuity and quality of care. · Develops and utilizes specialized knowledge of resources related to the needs of specific patient populations. · Facilitates efforts to obtain insurance coverage for hospital and community-based services. · Documents of the patient care plan, staff interventions and outcomes promptly and completely in the patient's medical record. · Completes statistical reports as required by the department and other programs. · Collaborate with interdisciplinary team to develop complex discharge plans prospectively and conduct transitional planning for patients moving to nursing home and other facilities. · Continues and encourages professional development through supervision, attendance at hospital and community conferences and mandatory in-service training with the intent of enhancing knowledge, sensitivity, and self-awareness about special issues related to culture, race, gender, religion or sexual orientation. · Participate in community education and support group activities as approved.

Job Description : · The Pathologist's Assistant provides services in Anatomic pathology as a member of a team under the direction and supervision of a qualified Pathologist. · Assists in the preparation and performance of human post-mortem examination. Principle duties and responsibilities: · Assists in the preparation and performance of human surgical specimen examinations, which includes: gross description and complete dissection of surgical specimens; selection and submission of tissue sections for microscopic examination (i.e., frozen and permanent sections for light, electron and fluorescent microscopy). · Responsible for the accessioning of specimens and assuring proper identification of · specimens in accordance to surgical pathology policies (Rejection/Rehabilitation Policy). Prepares containers and tissues cassettes. · Aids in the resident training and credentialing offering technical and teaching to residents in accordance with practice guidelines and the surgical pathology dissection manual. Assesses the resident's level of competency in handling specimens. · Responsible for gross photography of specimens as needed. Maintenance of files are completed by the assigned PA. · Responsible for tumor procurement for biorepository. Documentation and quality assurance indicators, and business transactions are completed by the assigned PA. · Aids in the quality assurance and quality control plan of the department. Primary responsibility in areas of specimen identification, frozen sections and in the quality control and maintenance of instruments and refrigerators in the work area. · Responsible for inventory of frozen section room and surgical pathology lab (E & R-6) · supplies.
