The Director of Product Management for Care Coordination is the single accountable product owner for Veradigm's care coordination and chronic disease management products and capabilities. This portfolio spans both the Provider and Payer sides of the business, where care coordination is delivered primarily through chronic disease management, care management, and population health workflows. Reporting to the Vice President of Product Management, this leader sets and executes the holistic care coordination product strategy, from short-term release planning through multi-year vision, ensuring every product decision is grounded in deep customer insight, healthcare market dynamics, and Veradigm's business objectives.
