Description: JOB SUMMARY: The primary purpose of this position is to plan, develop, organize, implement, evaluate, and direct the active treatment program in accordance with current existing Federal, State, and Local standards. The QIDP is also responsible for implementing Home of Hope, Inc. policies and procedures to assure that the individuals are provided with the opportunity to grow socially and emotionally. ESSENTIAL JOB FUNCTIONS: * Serve as the interdisciplinary team leader responsible for planning, developing, organizing, implementing, evaluating, and directing the active treatment program of each individual. Involve the individual/family in planning service programs when possible. Provide consultation to members of staff, community agencies, etc., efforts to solve the needs and problems of the individuals through the development of service programs. Assist all team members with annual individual plans and any IP addendum meetings. Will act as an advocate in protection of individuals' rights and responsibilities. Responsible to ensure compliance with HIPAA in all program areas. Ensure that rights to fair and equitable treatment, self determinations, individuality, privacy, property and civil rights, including the right to wage a complaint are well established and maintained at all times for the individuals we support. * Keep abreast of current Federal and State regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the ICF-MR. Implement and administer policies and procedures for the identification of medically related social and emotional needs of the individuals. Assist the individuals we support with integration into the community to help arrange various recreation and social activities based on the interest of the individual. * Develop and participate in regularly scheduled orientation and in-service training programs in relation to the social, emotional and medical needs of the individuals. Participate in discharge planning, development, and implementation of care plans and individual assessments. Inform the individual/family of the individual's personal and property rights. Maintain confidentiality of all pertinent individual care information to assure individual rights are protected. Regular monthly contact with parents and/or guardians. * Keep abreast of economic conditions/situations and recommend adjustment to assure the continued ability to provide quality individual care. Make written and oral reports/recommendations to the ICF-MR Administrator regarding areas of concern with the program. Work with the facility's consultants and implement recommended changes as required. Monitor and document all records per program policies and procedures. Assist in standardizing the methods in which work will be accomplished. Coordinate service activities with other departments as necessary. * Meet with the Administrator, on a regular scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services. Meet with the administration, advisory staff and other related departments in planning services. Recommend to the ICF-MR Administrator the equipment and supply needs of the individual services. Ensure that rights to fair and equitable treatment, self determinations, individuality, privacy, property and civil rights, including the right to wage a complaint are well established and maintained at all times for the individuals we support. Nurse Leader

The 4/5 West Observation Unit is a brand new unit at Parkview Randallia. This new department will be is a 20‑bed unit serving low‑acuity patients expected to have a length of stay of less than 48 hours, with an average stay of approximately 24 hours. The unit is divided evenly between 4 West and 5 West and functions as a short‑stay observation area with frequent admissions and discharges in a fast‑paced environment. Nurses care for patients admitted for brief observation and may also support after‑hours infusions, working alongside one patient care technician per floor. Summary Responsible to develop an overall view of patients, family and caregivers by establishing strong partnerships dedicated to optimal care. The NL facilitates the delivery of optimal patient care by the healthcare team through delegation, facilitation, collaboration, critical thinking and prioritization. The NL is accountable for the management of care and the care environment by ensuring effective communication through all members of the healthcare team. Independently plans and provides excellent professional nursing care for patients in accordance with physician orders, established hospital and departmental policies and procedures, American Nurses Association Standards of Clinical Practice, Indiana Nurse Practice Act, and the ANA Code of Ethics. Education Must obtain Bachelor of Science in Nursing (BSN) within three years of employment or will not be eligible to continue in the role. Coursework is completed outside of work hours. Licensure/Certification Valid Indiana Registered Nurse license required. Must have current CPR certification. Additional certifications may be required for specified areas. The Radiation Therapist in the Radiation Oncology Department applies ionizing radiation to a diverse population of patients in accordance with the prescription and instructions of the Radiation Oncologist. They must be capable of operating radiation-producing equipment utilizing radiation safety measures and be able to perform a series of complex and precise technical procedures. Core Responsibilities and Essential Functions: Participates in Quality Control Monitoring. Delivers consistent precise therapy to a designated treatment area. Demonstrates skill in patient assessment, performing procedures and giving specialty care to patients, considering the age of the patient and appropriately adopts care for age (adolescent, adult and geriatrics). Documents appropriately in Electronic Medical Record (EMR) Demonstrates a willingness to perform additional tasks and gives assistance when necessary to others. Recognizes own limitations and/or needs and seeks assistance as needed. Reports adverse reactions to physician immediately and follows policy and procedure (P/P) for documentation. Educates patient/family on expected side effects, use of blocks, wedges and other treatment aids. Follows P/P to ensure precise treatment per physician prescription.
