Physician - Long-Term Care & Transitional Care Units (Floating) New Graduates Accepted Locations: Franklin, Olivia, and Wabasso, MN (Floating required) Job Type: Full-time or Part-time Schedule: Monday - Friday, 9:00 AM - 5:00 PM (On-call rotation required) Work Setting: Long-Term Care (LTC) & Transitional Care Units (TCUs) Job Summary: We are seeking a dedicated and compassionate Physician to provide high-quality medical care for patients in Long-Term Care and Transitional Care Unit settings. This role requires the provider to float between multiple facilities and work collaboratively with an interdisciplinary healthcare team to deliver comprehensive patient-centered care. The ideal candidate will have experience in geriatrics, long-term care, or post-acute/rehabilitation settings, though motivated providers without prior LTC experience are welcome to apply. Key Responsibilities: Perform comprehensive initial assessments and ongoing medical evaluations for LTC and TCU patients. Develop individualized treatment plans based on patient history, condition, and long-term goals. Diagnose and manage acute and chronic medical conditions. Prescribe medications, order diagnostic tests, and adjust care plans as needed. Work closely with nurses, therapists, and social workers to coordinate and optimize patient care. Provide education to patients, families, and caregivers regarding treatment options and disease management. Maintain accurate, timely, and compliant documentation aligned with regulatory standards. Collaborate with palliative and hospice care teams as needed to support end-of-life care decisions.
