You will communicate regularly with physicians, therapists, social workers, and other team members to ensure coordinated, patient-centered care. A typical day in this role includes visiting multiple patients in their homes, documenting care and progress using an electronic medical record system, and adjusting care plans based on clinical findings. You will organize your schedule to balance skilled visits, follow-up checks, and coordination tasks, while maintaining timely and accurate documentation. Caseloads and visit numbers are managed to support safe and effective care, with a focus on quality outcomes and patient satisfaction rather than high-volume throughput.
