SUMMARY Promote and deliver the USRC Kidney Care Options Program to nephrology offices, hospital case managers, community partners, and USRC staff. Build and maintain internal and external clinical relationships, educate patients with advanced CKD and ESKD, coordinate care in preparation for initiation of or navigation between dialysis modalities, and support home therapy growth. Track and report program metrics and maintain documentation and communication across teams. Essential Duties and Responsibilities include the following. Other duties and tasks may be assigned. Optimal Start and Relationship Management: Strategically promote the Kidney Care Options Program to nephrology offices, hospital case managers, and USRC staff to drive patient referrals, expand home therapy pipelines, increase optimal access starts for in-center patients, improve transplant rates, and advance program awareness. Develop and sustain strong relationships with external and internal physicians/hospitals. Provide follow-up to Nephrologists regarding education provided to their patients. Options Education: Plan, schedule, and deliver group and individual Kidney Care Options classes for CKD Stage 4 and 5 patients and their families to support optimal management and modality decision-making. Under direct RN supervision, deliver Kidney Care Options education to all incident in-center patients within 90 days of admission. Collaborate with the in-center team to provide routine home therapy assessment and ongoing education for established patients. Conduct follow-up with educated patients at a prescribed cadence (dependent on CKD progression) to maintain engagement and ensure smooth transition to therapy. Participate in USRC team member training and events to ensure team-wide knowledge of the Kidney Care Options Program. Care Coordination: Monitor and track patients through the Kidney Care Options Tracker platform. Maintain communications throughout to in-center staff, home staff, and physicians on patient status. Report metrics to leadership at an established cadence. Serve as patient liaison to coordinate care needs including dialysis access placement, transplant referral initiation, insurance resource navigation, and transition planning to USRC facilities. Partnership: Collaborate with Value Based Care for optimal start of treatment modalities where applicable. Partner with Administrators, Social Workers, Home Therapy Registered Nurses, Regional Vice Presidents, Business Development, and Home Therapy Leadership to identify and pursue home growth opportunities. Lead and/or participate in strategic outreach events with physicians and community partners. Key Performance Indicators (KPIs): Documentation Adherence: Maintain compliance with required documentation standards. Patient Education: Complete prescribed number of KCO Education sessions Physician Engagement: Touchpoints with referring nephrologists, hospital case managers, or physician office staff. Qualifications/Requirements: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Requirements include: Bachelor's degree or equivalent work experience. Five years of experience in a healthcare organization working with patients and/or physicians in a clinical setting Knowledge of nephrology, End-Stage Renal Disease (ESRD), dialysis treatment or related field required. 1-year patient education and/or 1-year sales experience preferred. The Biomedical Technician is responsible for overseeing the overall operational condition, maintenance and repair of all water treatment equipment, all medical equipment and mechanical/electrical systems to ensure patient safety and the safety of all staff operating such equipment for their assigned clinic(s). ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties and tasks may be assigned. Demonstrate effective use of supplies and staff labor hours. Assist with purchasing as needed: place orders, perform weekly and month end inventory counts. Be familiar with the overall procedures used in Inventory Control (FIFO) and assist in the documentation and control of excess inventory. Perform duties as assigned to meet the patient care or operational needs of the clinic. Troubleshoot, repair and maintain equipment as recommended by the manufacturer. Repair, calibrate, test, clean and disinfect dialysis machines, water distribution loop and RO's according to protocol. Perform water quality checks to include but not limited to: free chlorine, total chlorine, and water hardness testing. Perform safety checks and tests on all pretreatment water equipment including but not limited to carbon tanks, water softeners, RO units, UV lamps and ultrafilters. Collect samples of RO water and dialysate for microbiology testing according to protocol. Collect water samples for AAMI analysis testing according to protocol. Disinfection of central bicarbonate system and mixing tank according to protocol. Disinfection of RO, distribution system, and hemodialysis machines according to protocol. Perform duties at all times within limitations established by and in accordance with company policy and procedures, applicable state and federal laws and regulations. Perform electrical safety on dialysis machines and related equipment. Be familiar with instruction manuals and operation for equipment in accordance with manufacturer's recommendations. Be capable of starting up and shutting down facility's equipment if necessary. Maintain established preventative maintenance programs and required support documentation. Maintain accurate maintenance records for the facility's equipment. Have knowledge of the function and safe operation of water treatment equipment and related mechanical and electrical systems. Address physical environment issues which could impact patient and staff safety. Mix bicarbonate solution according to protocol. Mix acid concentrate solution according to protocol. Assist in receiving, storing and stocking of supplies. Be familiar with all emergency equipment and emergency operational procedures. Use appropriate safety measures including personal protective equipment as necessary. Be familiar with OSHA regulations. Update Material Safety Data Sheets (MSDS) as required. Be aware of all contracts and contacts necessary for the emergency repair of the clinic's equipment and mechanical systems. Obtain quotes from reputable and approved contractors as needed to address physical environment issues. Present technical summaries in QAPI meetings. Maintain collaborative working relationship with Administrator(s). Partner with Administrator(s) to ensure clinic needs are met. Responsible for the support of the polysomnography (PSG) collection systems and related equipment. Assists with coordinating the expansion of services and plays a key role in site selection, equipment procurement, installation, and functionality. Responsible for computer upgrades, equipment troubleshooting and related issues, software upgrades, and data management. Assists with accreditation, sources of information, equipment maintenance, and supply ordering. Collaborates with vendors for new supplies and equipment. Provides feedback and assist with budget as necessary. Oversees data management, quality assurance, and accreditation standards. Assists with administrative duties such as payroll and scheduling. Facilitates staff communication and runs departmental meetings. Attends council meetings, tasks force, and committees as a representative. Possesses a basic knowledge of sleep disorders, electronics, electrical safety, with skill to utilize computers for patient documentation and data gathering for patient care. Ability to assist staff in performing sleep study procedures and may perform neurodiagnostic testing for applicable patient populations. May perform other scoring of respiratory related tests. Will complete review and final scoring of tests, as well as oversee review scoring completed by registered technologists when needed. Qualifications Associate degree required. Bachelor's degree preferred. Field of study preferred in an accredited program in Respiratory Care and/or Electroneurodiagnostic Technology, or other medical specialty. Minimum of five years' experience in sleep disorders is required. Supervisory experience helpful. Registry with the Board of Registered Polysomnographic Technologists (BRPT) as a Registered Polysomnographic Technologist (RPSGT). Or will consider registry with the American Board of Sleep Medicine (ABSM). Working in North Dakota (ND), must be licensed as a RPSGT. Obtain and maintain mandatory continuing education units (CEU) each year to maintain licensure and/or registry. Basic Life Support (BLS) certification is required within 60 days of employment.
