Conducts staff development programs or educational services to pharmacy staff, nursing staff, Oncologists, and other healthcare professionals. Serves as a subject matter expert in oncology, completing drug information questions for the oncology service line, reviewing Electronic Health Record care plans to ensure they are consistent with current national guidelines. Round with the Oncology Fellows, Nurse Practitioners, and Physicians to optimize therapy according to established national evidence-based guidelines and Bayhealth policy. This includes but is not limited to making recommendations regarding correct medications, monitoring outcomes, dose adjustments, and supportive care measures. Provides patient specific medication education using teach back method for new oral oncolytics and for intravenous chemotherapy as needed. Review oncology care plans within the Electronic Health Record. This includes new care plans as requested by Oncologists and a continual review of existing care plans for updates and appropriateness.
