Key Responsibilities Provide skilled nursing care to patients in the home setting Assess patients in both stable and urgent situations Develop, implement, update, and evaluate individualized care plans Maintain accurate and timely clinical documentation Educate patients and families on medications, treatment plans, diet, and home safety Coordinate care with physicians, caregivers, and the interdisciplinary team Supervise and guide support staff as needed Monitor patient progress and ensure continuity of care Use proper infection control practices at all times Assist with referrals and ensure payer compliance requirements are met Participate in team meetings and communicate effectively across care teams Schedule 13-week assignment 28 units per week No on-call No weekends Full-time available No guaranteed hours Must be willing to cover the entire DFW area Required Qualifications Current Registered Nurse license Current CPR certification Current driver’s license Reliable transportation and active insurance Homecare Homebase experience required OASIS experience required 1–2 years of RN experience in a clinical care setting 2 years of home health experience Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program Preferred Qualifications Bachelor of Science in Nursing (BSN) CHPN certification Ideal Background We are looking for an RN who is confident with: Home health case management Skilled nursing visits in the field Patient and family education Wound care and routine nursing procedures Venipuncture and IV care Coordination across multiple disciplines Independent scheduling, organization, and follow-through Benefits Benefits are available to full-time employees after 90 days of employment.
