Core duties include but are not limited to: set up and assemble various food for service, take food orders, and serve/deliver meals to the residents. Wash dishes, clean equipment and floors, follow industry standard sanitation practices. Prioritizes resident needs and practices customer service excellence in all professional interactions. Essential Duties and Responsibilities: 1. Waits on tables in a timely manner and ensure resident needs are met promptly and accurately. 2. Assists in general sanitation of the kitchen, sets up dishmachine, washes dishes and utensils, sorts silverware and cleans glasses. 5. RN- Registered Nurse (PER DIEM) Masonicare at Wesley Heights - Shelton, CT PER DIEM Summary of Position: Under the direction of the Supervisor of Assisted Living, assumes the professional responsibility and accountability for a group of residents, providing and coordinating care which is focused on achieving the residents optimal functional level of health within the framework of supportive and restorative care, health counseling and teaching, case finding and referral. Understands the Mission, Vision and Values of Masonicare, and Masonicare Assisted Living. Essential Duties and Responsibilities: 1. Performs initial and ongoing nursing assessments utilizing the nursing process to establish a comprehensive goal directed resident plan of services. 2. Develops, implements and evaluates resident plan of services. 3. Transcribes, implements and evaluates physician orders in a timely manner. 4. Provides education, supervision and counseling to resident and family regarding AL procedures and other care needs. 5. Provides coordination of the plan of services and maintains continuity of care with other professionals. 6. Documents accurate, pertinent and comprehensive information in a timely and concise manner. 7. Conducts supervisory visits of resident care assistants, companions and homemakers as required. 8. Arranges and participates in educational programs, inservices, meetings and workshops and utilizes current literature to enhance professional skills and knowledge. 9. Perform other duties as assigned. 10. Assessments, completed as often as necessary based on client's condition but not less frequently than every one hundred and twenty (120) days, and prompt action when a change in the client's condition would require a change in the client's service program. 11. Referral to appropriate professionals or agencies, whenever the client's condition necessitates, including the provision of current clinical information ensuring that if the client's condition is no longer chronic and stable, services of a licensed home health agency are engaged or other appropriate arrangement are made. 12. Planning for clients who shall no longer receive or require the services of the assisted living services agency. 13. Clinical Dietitian (PER DIEM) Masonicare at Bishop Wicke - Shelton, CT Per Diem The Masonicare at Bishop Wicke - 120 bed SNF The Clinical Dietitian is responsible for providing clinical nutrition services within the regulatory agencies including Masonicare and federal, state and local agencies. Essential Duties and Responsibilities: Assure comprehensive initial, change of condition and annual nutrition assessment process/Resident Assessment Instrument (RAI), discharge summary is initiated, planned, developed, executed and monitored accurately and within the time frames specified by state, federal and/or accreditation requirements. Plan, organize, schedule, document and direct individual and group resident and family education experiences. Use age appropriate materials and methods for determining comprehension and potential for compliance. Monitor dietary profiles, intake records and other internal documentation/tracking processes. Communicates specific nutritional needs to dining service associates, dining service coordinator, culinary staff and management. Participates in ongoing continued nutrition education. Participates as a member of the interdisciplinary care team to promote optimal nutritional care for our patients/residents. Participates in QI projects as assigned for food & nutrition and Masonicare quality improvement committee. Provides guidance and support for the Diet Technician for Nutrition Care Process. 9.
