Outreach Contractor Boston, MA 3 Months Short Description: Field based peer outreach supporting justice-involved veterans with stabilization and resource connection. The Contractor provides field based, client focused peer outreach and engagement services to service members, veterans, and their families, with a particular emphasis on justice involved veterans. Services support suicide prevention, justice intervention, housing system navigation, recovery support, and connection to appropriate federal, state, municipal, and community based resources. The role is non-clinical and limited to peer support, outreach, coordination, and resource navigation within the defined contractual scope. The Contractor does not provide legal advice, clinical treatment, or benefits determinations. Servicesare delivered in community based settings and must comply with HIPAA, confidentiality standards, and Personally Identifiable Information (PII)protections. As it relates to military service members, veterans and their families, with particular emphasis on justice involved veterans: Conduct structured intake assessments capturing relevant information including service history, physical ability, trauma exposure, substance use history, suicidal ideation, mental health history, stabilization needs, and other pertinent information in accordance with EOVS documentation standards. Assist veterans in identifying immediate needs, stabilization priorities, and short-term goals. Support the development of Individual Recovery Plans (IRPs) or comparable goal-oriented service plans focused on stability, rehabilitation, community reintegration, and reduction of systemic barriers.
