Position Types Available: Full-Time, In Office-Hybrid The Nurse Care Manager is a highly qualified health care provider who, through education and experience, possesses a distinct body of knowledge and skills and applies the art and science of his/her profession. The Nurse Care Manager is in charge of helping implement Family Caregiver Services from a clinical standpoint. He/she applies nursing knowledge in assessing, implementing, and evaluating client responses to actual or potential health problems. He/she is responsible for (1) providing direct nursing care as prescribed by the physician to acutely ill, and/or chronically ill clients in their homes; (2) recording clinical and progress notes; and (3) coordinating other client care activities/services to provide the highest quality of client care in the geographic service area to which he/she is assigned. The Nurse Case Manager will be able to complete and assist with all Pediatric Assessment Tools (PAT) when required for the program. The Nurse Care Manager will be able to assist with all clinical duties, including but not limited to skilled assessments to determine eligibility, start of care, supervision of CNAs and/or LPNs, and recertification visits. Will generate Plans of Care and Care Plans and assist with all things pertaining to clinical operations and visits. He/she works under the supervision of and receives clinical direction from the Director of Nursing. The nurse care manager will also help with administrative tasks such as but are not limited to the coordination of services, contacting physician offices, requesting outstanding documentation, and reviewing the completeness of records. At COFK, the Nurse Care Manager (RN) is expected to be proficient and efficient with the use of an EMR system and multiple technological platforms to provide the highest quality of care to the patients and families we serve. Nurse Care Manager responsibilities include: Ability to effectively present information to clients and employees and community. Knowledge of principles and scope of speech therapy. Thorough knowledge of speech therapy theory and practice, home health care practice, communication skills and case management. Basic knowledge of Windows operating system and ability to advance computer skills as required.
