Job Description Job Description Job Type: Full-time, PRN Salary Range: $45-$55/hr based on experience Setting & Population Served Our facilities function as Skilled Nursing Facilities (SNFs) that offer: Long-term care for residents needing continuous skilled assistance and compassionate daily support. Short-term, sub-acute rehabilitation for individuals recovering from surgery, illness, or injury. Collaborative care through a team-oriented approach involving nursing, therapy, and medical staff working in unison. A diverse patient demographic that includes those with orthopedic, neurological, cardiopulmonary, and post-acute needs. The patient mix may vary slightly across different facilities. As a therapy professional, your role will be to assist patients in achieving functional improvements and enhancing their quality of life within a nurturing, patient-focused environment. Position Summary The Physical Therapist (PT) delivers direct rehabilitation services in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards. Education, Experience, and Licensure Requirements Graduation from an accredited physical therapy program, holding a BSPT, MSPT, or DPT (those graduating in 2003 or later must possess an MSPT or DPT). Current registration/licensure in the applicable state, with an obligation to maintain an active license in good standing throughout employment. Preferred one (1) year of experience in a post-acute care or similar setting. CPR certification required upon hire or to be obtained during orientation, with the necessity to keep it current throughout employment. Specific Job Requirements Engagement in community health initiatives/projects as appropriate. Proficiency in Microsoft Word, Excel, and email communication. Collaboration with patients, families, and support departments to effectively address patient needs. Consistent demonstration of proper body mechanics. Ability to make independent decisions when necessary. Familiarity with practices, procedures, and regulations governing operations in a post-acute care setting. Implementation and interpretation of departmental programs, goals, objectives, policies, and procedures. Competency in all relevant areas, including patient rights, safety, and sanitation. Maintenance of professional relationships with colleagues, vendors, and others. Confidentiality of all proprietary and sensitive information must be upheld. Understanding and adherence to organizational policies, including those related to harassment and compliance. Exhibition of integrity and professionalism by following the organization’s Code of Conduct and completing mandatory training. Essential Functions Adherence to physical therapy treatment plans for patients. Establishment, assessment, and modification of realistic, measurable, timely, and functional goals. Supervision and evaluation of care provided by PTAs, Rehab Aides, and PT and PTA students. Timely and appropriate documentation of patient care. Accurate and effective use of therapy software. Provision of excellent customer service and a positive demeanor towards patients. Assistance in the evacuation of patients when necessary. Consistent attendance and reliability. Ability to concentrate, apply reasoning skills, and exercise good judgment. Effective communication and collaboration within an interdisciplinary team. Physical capability to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours.
