Job Summary Under supervision of a Physical Therapist, implements specific treatment programs for neonatal through geriatric patients according to accepted physical therapy practices and principles. Home Based Physical Therapist

Job Summary The OP Home-Based Physical Therapist is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions consistent with the position's qualifications, professional practices and ethical standards. The incumbent shall also demonstrate accountability for and contribution to program development, quality improvement, and problem solving and productivity enhancement in a flexible interdisciplinary fashion. Performance: Position Specific Essential Functions Conducts evaluations and treatments as referred and prescribed by the physician to patients ranging in age from adolescents to geriatric. Provides therapy to include but not limited to muscle strengthening and stretching, gait training, ADL training, use of adaptive and other equipment and management of orthotic and prosthetic devices. Performs initial and periodic evaluations and progress notes of patient's physical therapy and rehabilitation needs. Formulates appropriate treatment plans including short- and long-term goals for rehabilitation, evaluating progress or lack of progress in achieving these goals. Completes accurate, timely, and compliant clinical documentation (evaluations, daily notes, progress reports, discharge summaries, etc.) according to department and payer guidelines for outpatient services. Instruct patients and caregivers in activities to be carried out between treatments, using clearly written, concise instructions to reinforce verbal instructions when appropriate. Communicates regularly with referring physicians and other healthcare team members; provides updates on patient progress and coordinates any changes to patient's plan of care. Independently manages appointment scheduling including efficient travel routes between patients' homes. Maintains reliable transportation and carries portable therapy equipment and supplies for use during therapeutic interventions. Maintains department policies, procedures, objectives and quality improvement programs, safety environmental and infection control; assists with maintenance of departmental supplies and equipment; participates in departmental program development. Independently manages productivity expectations. Monitors patients' authorizations limits and insurance coverage and communicates with the Director/Manager any scheduling, insurance or patient concerns. Performs other duties as assigned. Registered Dietitian

PURPOSE AND SCOPE: Provides nutritional services for the facility's in-center and home patients as applicable in order to maximize the patient's nutritional status and improve clinical outcomes. Functions as a member of the interdisciplinary healthcare team, supporting the FMCNA commitment to the Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) activities. Actively participates in process improvement activities that enhance the likelihood that patients will achieve their individualized patient-specific goals as determined by the patient's physician. PRINCIPAL DUTIES AND RESPONSIBILITIES: Assesses patient's knowledge of diet and kidney disease and provides education appropriate to patient's learning needs regarding the relationship of diet and kidney disease and the impact on the treatment process. Provides patient specific detailed education to patient/family/caregiver regarding nutritional status, adequacy measures and results, fluid status, and all nutrition related lab parameters pertaining to their disease process. Completes nutritional assessment and participates in the interdisciplinary comprehensive assessments. Participates with the interdisciplinary team in the development of a written, individualized comprehensive plan of care that specifies the services necessary to address the patient's needs, in accordance with established Federal and, where applicable, state licensure guidelines. Calculates diet prescription according to standard of practice. Completes progress notes on all chronic dialysis patients according to established FMCNA guidelines. Maintains complete and accurate documentation of all appropriate information in medical record according to established FMCNA policies and Federal and state regulations. Provides patient with ongoing nutrition assessment and outcome-oriented nutrition counseling necessary to assist patient in achieving and sustaining an effective nutritional status. Identifies malnourished patients as well as patients at risk for malnutrition and works collaboratively with interdisciplinary team to identify appropriate interventions, resources or solutions. Reviews each patient's albumin level and body weight at least monthly. Implements plan of care with involvement of patient/family/caregiver. Monitors adherence and response to nutrition therapy, adjusts therapy as needed to achieve and sustain an effective nutritional status, documents why the patient was unable to achieve the goals and implements the plan of care to address issues impacting the patient's ability to achieve the goals. Collaborates with the interdisciplinary team on Urea Kinetic Modeling recommendations to achieve the physician's prescription of Kt/V. Collaborates with the interdisciplinary team on management and recommendations for mineral metabolism and renal bone disease, anemia and iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation status and sodium/volume control. Adheres to FMCNA algorithm and medical record policies. Reports on nutrition QAI results and participates in the interdisciplinary QAI program. Collaborates and coordinates with team members in provision of Pre-ESRD education as appropriate. When appropriate, works with Case Manager to coordinate plan of care for patient education and identifies candidates to refer to physician for nutritional supplements, as per disease management agreements. Reviews nutrition related lab results: Counsels patient and/or caregiver and formulates appropriate action. Recommends treatment changes to the interdisciplinary team as appropriate. Communicates with physician and/or facility staff regarding lab results as appropriate. Communicates lab results and recommended treatment changes to providers of patient care in nursing facilities (i.e., hospitals, short term and long-term care facilities). Participates in pertinent staff meetings (i.e., general staff, quality improvement), Care plan meetings, and others as applicable. Consults with FMS Corporate Dietitian regarding nutrition concerns as needed. Maintains and improves knowledge and skills for a competent and innovative practice. Maintains dietetic registration and continuing education hours as specified by American Dietetic Association and state licensure regulations where applicable. Collaborates with interdisciplinary team, ensuring that all work areas are safe and clean. Other nutrition management duties as assigned. Performs other related duties as assigned. Acute Inpatient Registered Nurse - RN - Dialysis

PURPOSE AND SCOPE: The Inpatient RN provides direct patient care in accordance to company policies and procedures which includes FMCNA compliance programs and contracted hospitals' policies and procedures. Areas of practice are diverse, including, but not limited to, hemodialysis, peritoneal dialysis, continuous renal replacement therapies, apheresis, and ultrafiltration. The Inpatient RN will partner with the hospital to provide quality care to the patient and deliver care that provides a positive patient experience. PRINCIPAL DUTIES AND RESPONSIBILITIES: Under limited supervision, applies considerable knowledge to perform a broad range of tasks which include but are not limited to direct patient care in multiple modalities not limited to hemodialysis, peritoneal dialysis, ultrafiltration, continuous renal replacement therapy, and Apheresis. Provides supervision and direction of Patient Care Technicians and clerical staff; provides direction and education to ICU staff for CRRT. Responsible for direct patient care of assigned patients in the Inpatient Renal Replacement and Apheresis Services setting and providing the appropriate treatment technique for assigned patients as prescribed in all modality procedures, as appropriately defined - hemodialysis, peritoneal, continuous renal replacement therapies, ultrafiltration, and Apheresis. Assesses patients' responses to treatment therapy making appropriate adjustments and modifications to the treatment plan as indicated by the appropriately credentialed physician. Communicate problems or concerns to the Inpatient Program Manager, appropriately credentialed physician, patient hospital primary nurse and others as indicated. Delivers safe, effective care in a timely an efficient manner. Gives organized, concise shift reports for patients on assigned shifts. Apply high level thinking skills to shift patient schedules to meet needs of more critical patients due to labs, MD order, or patient assessment. Responsible for the implementation, administration, monitoring, and documentation of patient's response to prescribed intradialytic transfusions, including appropriate notification of adverse reactions to physician and appropriate blood supplier. Collaborates and communicates with physicians and other members of the healthcare team to interpret, adjust, and coordinate daily patient care plan to ensure continuity of care. Initiates and communicates to FMS facility nursing staff and appropriate hospital personnel as needed. Accurately documents all treatment information in the individual patient record (ACES) and the hospital record, as needed. Cleans and disinfects dialysis machine surface, chair, equipment, and surrounding areas between treatments according to inpatient renal services policies and procedures. Conducts all tasks necessary for preparation for dialysis treatment and documents where appropriate. Performs all required pre-treatment dialysis machine alarm testing. If applicable, initiates Solution Delivery System. Operates all related equipment appropriately and safely and provides minor trouble shooting when necessary; efficiently utilizes supplies to prevent waste. Understands, conducts, and documents appropriately Dialysis/Apheresis machine safety tests/alarm tests, equipment calibration, dialysate testing, machine safety tests, functional testing, and internal and external disinfection on all water & dialysis machines, and complies with the documentation/notification standards per FMS policies. Completes and documents ongoing participation in QAI activities. Participates in staff meetings as scheduled. Participates in staff training and orientation as assigned.
