POSITION SUMMARY/RESPONSIBILITIES The Behavioral Health (BH) Social Worker assists Community First members with their Behavioral Health needs by assessing and identifying unmet needs and coordinating care, utilizing Covered Services and community resources with the goal of optimizing the Member's well-being, independence, and integration in the community. The BH Social Worker collaborates with other teams in Population Health Management (PHM) to ensure that the member’s physical, behavioral, functional, and psychosocial needs and preferences are met.
