Community Health & Equity Specialist Salary Range: $95,742.40 – $143,603.20 annually Schedule: Monday – Friday | 8:00 AM – 5:00 PM Job Type: Full-Time Position Overview We are seeking a Community Health & Equity Specialist to support population health and health equity initiatives that improve outcomes for diverse communities. This role plays a key part in the development, implementation, and evaluation of health programs designed to address health disparities and promote equitable healthcare access. Working under the direction of the Population Health and Equity Manager, this position supports the organization’s population health strategy, program monitoring, and regulatory compliance , including Medi-Cal regulations and NCQA Population Health Management accreditation standards . The ideal candidate will bring experience in community health, population health programs, health promotion, and health equity initiatives , along with strong project coordination and communication skills. Key Responsibilities Assist in the planning, implementation, and evaluation of population health programs Develop and support health promotion initiatives addressing health disparities Ensure programs meet the cultural and linguistic needs of diverse populations Compile and analyze qualitative and quantitative data to evaluate program effectiveness Prepare clear reports, summaries, and presentations on program outcomes Develop health education materials, newsletters, presentations, and website content Support member communications and interactive health promotion initiatives Build and maintain relationships with community organizations, healthcare providers, vendors, and coalition partners Plan and facilitate meetings, trainings, focus groups, and surveys Develop and maintain department workflows and standard operating procedures Interpret contractual and policy requirements and apply them to program activities Assist with additional projects and initiatives related to population health and equity Essential Functions Work respectfully with diverse populations and communities Maintain strong relationships with internal teams, providers, and community partners Collect, analyze, and report health program data and performance metrics Stay current on healthcare best practices and managed care regulatory requirements Manage multiple projects simultaneously while meeting deadlines Deliver excellent service to members, providers, and internal stakeholders Communicate professionally via presentations, reports, email, phone, and meetings Ensure compliance with organizational policies, regulatory requirements, and internal controls Minimum Qualifications Education Master’s degree in a health or related field , OR Bachelor’s degree with 3 years of experience in population health, community health education, or health equity Experience Minimum 2 years of relevant project management or health promotion experience Preferred Qualifications Master’s degree with specialization in Public Health or Community Health Education Degree from a program accredited by the Council on Education for Public Health (CEPH) Certified Health Education Specialist (CHES) credential Skills & Competencies Strong project management and organizational skills Ability to analyze health data and program outcomes Knowledge of population health, community health, and health equity strategies Strong communication and presentation skills Ability to work independently and collaboratively Experience working in diverse community settings Proficiency with Microsoft Office (Excel, Word, Outlook, PowerPoint) Ability to handle confidential health information with professionalism and discretion Physical Requirements Extended computer and desk work Frequent keyboard and mouse use Regular verbal and written communication Occasional lifting of materials up to 30 pounds Periodic walking and standing Work Environment This role operates in a professional office environment with frequent collaboration with healthcare teams, community organizations, and internal staff.
