Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine (ABMS or AOA). An active, unrestricted medical license in your state (or willingness to obtain one). Strong interpersonal skills and a passion for building meaningful patient relationships. A collaborative mindset, fully committed to team-based care. Comfort with value-based care practices and electronic medical records (EMR). Compliance with Tuberculosis (TB) screening requirements. Preferred: Active DEA license. Medicare and Medicaid provider credentials. Two or more years of experience in primary care or a value-based care environment.
