Performs home safety inspection, patient assessment, medication reconciliation, point-of-care testing, telehealth facilitation, and equipment setup to support safe transition of care and reduce readmissions. Position Responsibilities: Conducts comprehensive in-home patient assessments, medication reconciliation, and administers treatments within the paramedic scope of practice. Identifies clinical changes and documents plan of care. Completes accurate clinical documentation in the EMR/ePCR. Responds to quality assurance and medical oversight requests. Responsibilities Under general supervision, carry out general labor responsibilities promptly and efficiently.
