Sibley Memorial Hospital is a non-profit, full service 318 bed acute care community hospital serving the Washington, DC area. This growing program focuses on high needs geriatric home-limited patients and provides high-touch, comprehensive primary care. Collaborating with the Medical Director and other team members, the Nurse Practitioner (NP) will provide medical care for a small panel of home bound and medically complex older adults. The CCT helps clients connect with resources to better manage their conditions, improve health outcomes, and reduce preventable hospital visits. The CSW completes psychosocial assessments, provides education, advocacy, counseling, and support, and helps clients connect to behavioral health services. This role maintains a caseload and works closely with Community Health Nurses (CHN) and Community Health Workers (CHW) to assess psychosocial needs and determine appropriate emergent behavioral health services. The CSW also supervises the CHW embedded in the Emergency Department. MAJOR RESPONSIBILITIES Conduct comprehensive psychosocial assessments Complete home visits as needed Provide education, advocacy, counseling, and support Manage assigned caseload Assist clients with behavioral health treatment and community resource connections Support CHNs and CHWs in assessing psychosocial needs and determining appropriate care Maintain current knowledge of community resources Coordinate with medical providers and care team members to support comprehensive care Complete accurate and timely documentation Supervise the CHW embedded in the Emergency Department ADDITIONAL RESPONSIBILITIES Perform duties in accordance with hospital core values and the Language of Caring Attend required meetings, in-services, and continuing education Participate in continuous improvement efforts as needed Maintain effective communication with supervisor and coworkers Maintain acceptable attendance and punctuality Support the hospital’s mission and patient care goals SKILLS & ABILITIES REQUIRED Ability to respond to inquiries or complaints from patients, families, staff, and agencies Ability to present information effectively to patients, families, hospital staff, community resources, insurance companies, and case managers Proficiency in Microsoft Word EDUCATION & EXPERIENCE Graduate of an accredited college or university; MSW required Two or more years of experience in public health, behavioral health, or a related field Experience in acute care and/or community settings preferred Experience working with individuals with multiple comorbidities and mental health/substance use conditions preferred LICENSES & CERTIFICATIONS LCSW-C required CPR certification required within 60 days of hire and maintained thereafter Valid driver’s license, clean driving record, and reliable transportation required Salary Range: Minimum 34.62/hour - Maximum 57.19/hour.
