Collaborating with the Medical Director and other team members, the Nurse Practitioner (NP) will provide medical care for a small panel of home bound and medically complex older adults. The NP will be responsible for evaluating and treating patients receiving primary care at home, coordinating their care with other team members and specialists, and participating in weekly inter-disciplinary team meetings. Three to five years NP inpatient or outpatient practice preferred. General medical experience with geriatric, medically complex and high acuity patients desired Part time, 20 hours clincial and 4 hours admin NP Licensure in the State of Maryland required. Must have a working vehicle to travel to patient's homes. CPR Certification required. 3 years’ experience preferred in primary care or acute/urgent care.
