SUMMARY The role of the Health Care Navigator is to drive post-acute and long-stay census results for the Toledo Ohio Otterbein ministries. The position will work on a complementary basis with the Senior Lifestyle Communities; Sunset House, Sunset Village, Pemberville & Marblehead– along with Otterbein Small House Neighborhoods; Monclova, & Perrysburg. With a specific focus on: Otterbein Perrysburg, Otterbein Monclova, Otterbein Sunset House, Otterbein Sunset Village, and Otterbein Pemberville.
