The Central Alabama Veterans Healthcare System (CAVHCS) located in Tuskegee, AL is searching for a BE/BC Internal Medicine/Family Medicine Physician or completion of a Geriatric Fellowship to join their leadership team as the Medical Director for the Geriatrics Patient Aligned Care Team (GERI-PACT). The Medical Director for GERI-PACT of the Outpatient Clinic is accountable to the ACOS/GEC&R or designee for the coordination and oversight of clinical and administrative activities in the outpatient clinic. The physician is responsible for performance improvement activities in the clinic. He/she directs and supervise clinical activities of the Geriatrics Primary Care providers. The physician devotes 80% of time to direct patient care, serves as a role model for peers. The physician is responsible for the oversight and coordination of process improvement initiatives, assuring teamwork, and addressing customer service issues in the clinic. The position includes management and administrative responsibilities. Responsibilities include but are not limited to: Provide comprehensive assessment, diagnostic work-up and clinic intervention, including comprehensive patient assessment, health history, physical examination, and patient education to Veterans in the Geriatric Primary Care team setting; outpatients include both scheduled and unscheduled Veterans. Functions as an integral part of a Geriatric Primary Care multi-disciplinary team to coordinate, facilitate, and provide, patient care and services. Discriminates between normal and abnormal findings to establish a diagnosis and initiate appropriate response to emergency health problems. Initiates appropriates referrals and consultation to other clinical services. Order and evaluate diagnostic tests as appropriate, recommend treatment and/or referral. Validates the effectiveness of previously implemented medical treatment. Initiate, evaluate, and change orders for medication. Include patient and family in planning care and establishing realistic measurable goal for health maintenance/improvement. Record/document health appraisal information, clinical assessment, treatment decisions, plan of care, ongoing management and therapeutic outcomes, order medications, diagnostic tests and referral consultations where needed for every patient encounter, recording will be accomplished using the electronic comprehensive patient record system (CPRS), and will be completed on the day of the encounter. Promote all PACT initiatives. Coordinate clinic schedules, including provider and clerical staff leave requests. Develop and administer performance plans and appraisals for subordinates as required by VA regulations. Work Schedule: Full Time. 8 a.m. - 4:30 p.m. (Monday - Friday). Responds and coordinates response of others to Code Strokes in the hospital/ED. Recognizes trends in stroke care and explores stroke/neurosciences care process improvements. Assists in the development and revision of standards of care/guidelines/clinical pathways/order sets for stroke/neuroscience patients, incorporating an interdisciplinary approach, utilizing best practices, and research findings. Responsible for collecting and recording data required to achieve the benchmark targets, specifically STK Advanced Primary Stroke Center measures. Monitors compliance with regulatory guidelines, quality guidelines, and Advanced Primary Stroke Center guidelines. Works with nursing, rehabilitation and other services to ensure comprehensive patient education and preparation is achieved and maintained, on an ongoing basis. Assists in coordinating care of the Stroke/Neuroscience program patients through collaboration with physicians, the stroke coordinator, nursing, case management, social workers, therapists, radiology, lab, dieticians, pharmacy, and all other members of the healthcare team through team meetings and/or one-on-one meetings to ensure a quality care-delivery process for patients across the continuum. Participates in staff education on an individual and/or group basis. Provides support to the Stroke/Neuroscience team for continual learning. Coordinates with educators in the development of education programs for staff orientation, annual education, and yearly competencies.
