Geriatrics Medical Director- PACE Position

Job Description The PACE KC Medical Director is responsible for the delivery of member care, clinical outcomes, and the implementation, as well as oversight of the quality improvement program for PACE KC. The PACE KC Medical Director serves as the lead in the design of the medical components for PACE KC and as a liaison to the medical community on behalf of PACE KC. As a physician member of PACE KC’s team, the Medical Director must meet all federal and state licensing laws, certification and regulations necessary. The PACE KC Medical Director serves as the consultant and resource to the Primary Care Physician(s), on-call physicians and Nurse Practitioners. The PACE KC Medical Director will perform both administrative and direct patient care duties. Under the direction of the PACE KC Executive Director and the Health Chief Health Officer, the PACE KC Medical Director shall facilitate the development of an operational bridge between PACE KC and Health center and help establish a service line for Geriatric Care at Health center. These activities will include but is not limited to: Establishing and maintain an operational bridge between the two organizations including a referral system between the two organizations Leading or facilitating the development of a primary care service line for the care of Geriatric patients at Health center. This includes the clinical education and training (including proficiency in the management of Medicaid Programs) of a core care management team to ensure long-term sustainability of the program.
