Family Nurse Practitioner – (Float Position)
Pay: From $75.00 per hour Job description: Job Title: Nurse Practitioner – (Float Position) Location: Jacksonville, FL (Must be willing to float to Macclenny, Callahan, and Jacksonville clinics) Hours: 30–38 hours per week Schedule: 2–3 weekdays plus 6 out of 12 weekend shifts (Saturday/Sunday) per month Clinic Hours: Monday – Friday: 8:30 AM – 7:30 PM Saturday: 9:00 AM – 5:30 PM Sunday: 9:00 AM – 4:30 PM Additional Requirements: Holiday rotation required Position Overview Nurse Practitioner to provide high-quality, evidence-based primary and family care services across multiple clinic locations in the Jacksonville, Macclenny, and Callahan areas. This float role is essential to supporting clinic operations and ensuring patients receive timely, comprehensive care. The Nurse Practitioner will work collaboratively with a dedicated clinical care team to assess, diagnose, treat, and manage acute and chronic health conditions while promoting preventive care and patient education. This position reports directly to the Practice Manager and plays a vital role in improving overall patient health outcomes. Key Responsibilities Clinical Care Provide comprehensive primary and family care including assessment, diagnosis, treatment, and management of acute and chronic conditions Perform routine physical exams, health screenings, and diagnostic evaluations Develop and implement individualized treatment plans Provide health counseling, preventive care, and chronic disease management Encourage patient engagement in their healthcare and wellness Patient Management & Care Coordination Coordinate care with physicians and other healthcare professionals Conduct follow-ups and ensure continuity of care Promote preventive care and health education Support behavioral health and wellness initiatives Administrative & Operational Duties Open and close clinic as needed Maintain clinic cleanliness and infection control standards Manage inventory and supplies Verify insurance eligibility and collect payments Document patient care accurately using EMR systems Required Qualifications Master’s Degree from an accredited Family Nurse Practitioner program Active Nurse Practitioner license in the State of Florida, in good standing National Board Certification as a Family Nurse Practitioner Active and valid NPI number Current BLS certification from AHA or American Red Cross Minimum of 1 year of recent primary care experience preferred DEA registration (or willingness to obtain during onboarding) Strong clinical decision-making and patient care skills Ability to work independently and collaboratively Strong communication, organization, and time management skills Proficiency with Electronic Medical Records (EMR) systems Work Environment & Additional Requirements Must be willing to float between Jacksonville, Macclenny, and Callahan clinic locations Must be willing to work weekends and occasional holidays Safety-sensitive position Ability to work autonomously without direct supervision Strong adaptability and flexibility in a dynamic clinical setting The Nurse Practitioner will work collaboratively with a dedicated clinical care team to assess, diagnose, treat, and manage acute and chronic health conditions while promoting preventive care and patient education.
