Key Responsibilities Deliver comprehensive, patient-focused primary care services within a value-based care model . Manage a patient panel of approximately patients per day . Conduct visits averaging 20 minutes for regular appointments and 40 minutes for new or complex cases. Collaborate with a dedicated care team including a Medical Assistant, Scribe, and Social Worker . Utilize on-site lab services with phlebotomy support and access to RubiconMD for virtual specialty consultations. Maintain accurate documentation using Canopy and Greenway EMR systems . Participate in community health initiatives such as wellness classes, exercise groups, and social engagement activities. Schedule & Environment Schedule: Monday Friday, 8:00 AM 5:00 PM Clinic Model: 3 5 providers per site (mix of MDs and NPs) Transportation Support: Patient pickup service within 5 miles of the clinic Community Engagement: Regular on-site patient wellness events (yoga, dance, spa days, and more) Qualifications Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.)
