Key Responsibilities\n \n Conduct comprehensive assessments of seniors' psychosocial needs, utilizing tools to analyze emotional, mental, and social factors influencing their well-being. \n Collaborate with seniors and their families to develop personalized intervention plans tailored to their individual goals, preferences, and available community resources. \n Provide ongoing counseling and emotional support, assisting clients and their families in managing life transitions, such as adjustments to living situations, health challenges, or loss of loved ones. \n Champion clients' rights by advocating for their access to essential resources and services, ensuring they receive adequate support within the community. \n Engage with interdisciplinary teams—including nurses, caregivers, and other healthcare professionals—to create a cohesive care approach and address holistic client needs. \n Conduct regular follow-up visits, both remotely and occasionally on-site, to monitor clients' psychosocial status, ensure safe living conditions, and address emerging issues. \n Facilitate productive communication among family members and other stakeholders involved in the client's care, addressing varying perspectives and ensuring the best outcomes. \n Provide education on coping strategies, available community programs, and support networks to enhance the clients' social engagement and overall quality of life. \n Maintain up-to-date documentation and records of client assessments, treatment plans, progress notes, and relevant communications, ensuring compliance with regulations and ethical standards. \n Stay abreast of current best practices in social work and aging-related issues, actively incorporating new knowledge into your practice. \n Patient Population / Client Focus\n The primary demographic for this role includes seniors and their families who face various challenges associated with aging.
